肺主宣降与“肺为水之上源”相关性的实验研究及其应用探讨

摘　　要：【目的】探讨肺气的宣降运动与"肺为水之上源"之间的相关性,为中医治疗肺系疾病提供更多思路。【方法】豚鼠85只随机分为对照组(40只)和哮喘组(即肺失宣降组,采用卵蛋白致敏法造模,45只),Bradford法检测各组豚鼠尿AQP-2(水通道蛋白2)含量,放免法(RIA)检测血浆ADH(抗利尿激素)浓度。【结果】与对照组相比,哮喘组豚鼠小便量显著减少(P<0.05)、血浆ADH及尿AQP-2均显著升高(P<0.05、P<0.01)。【分析与结论】"肺为水之上源"的功能主要由肺气的宣降运动体现和完成,二者有密切相关性,其机制是哮喘(肺失宣降)状态下,ADH和AQP-2合成和释放量明显增多,抑制豚鼠体内水液代谢速度,并导致小便量减少。提示中医治疗哮喘、肺气肿等以肺失宣降为主要病理表现的疾病时,在使用常规宣发肺气、肃肺降气法的基础上,如患者伴有小便减少、浮肿等症状,在辨证论治基础上,可辅助使用行气、利水、通利小便法加强疗效,可选用方药有五苓散、五皮饮、实脾散、越婢加术汤等。[Objective]To discuss the correlation between the ascending,descending and clearing movements of the lung qi and ＂the lung is the upper origina of water＂, so as to supply more ways for TCM clinics to deal with lung diseases. [Methods] 85 guinea pigs are divided into two groups, the blank group and the control group （athma group）. Using RIA method to examine the density of ADH in the plasma and Bradford method to examine the content of AQP-2 in the urine of the guinea pigs and observe the change of unrine. [Results]Urine quantity in the control group is obviously less than that of the blank group （P0.05）, ADH density in the plasma of the control group is obviously higher than that of the blank group （P 0.05）, AQP-2 content in the urine of the control group was also obviously higher than that of the blank group （P 0.01）. [Analysis and Conclusion] The function of ＂the lung is the upper origination of water＂ is presented and performed by the ascending, descending and clearing movements of the lung qi, they have close correlation. The ascending of the density of ADH in the plasma and the content of AQP-2 in the urine is one of the pathophysiologic basis of the slow water metabolism and the reducing of urine under the condition of the asthma. Meanwhile, the result mentioned that except the normal method of clearing the lung and descending the lung qi or ascending the lung qi, methods of activating qi and promoting the circulation of water should be considered in the TCM treatment process of asthma, emphysema, etc. Formulas such as Wulingsan, Wupiyin, Shipisan, yuebijiazhutang could be considered.
