鼻内镜下YAG激光鼻腔泪囊开窗术82例临床观察

摘　　要：目的观察鼻内镜下YAG激光鼻腔泪囊开窗术治疗慢性泪囊炎的治疗效果。方法对2002年9月至2009年12月牡丹江医学院第二附属医院耳鼻喉科收治的82例(106眼)慢性泪囊炎患者行鼻内镜下YAG激光鼻腔泪囊开窗术患者的临床资料进行回顾性分析。结果术后75例患者溢泪、溢脓症状均消失;泪道冲洗通畅。其中5例术后1个月溢泪复发,发现泪总管堵塞,补行泪总管YAG激光治疗后症状消失。另2例开窗口上皮化程度不理想。所有患者均达临床治愈标准,随访3～24个月,无一例复发。结论鼻内镜下YAG激光鼻腔泪囊开窗术治疗慢性泪囊炎手术疗效肯定,术后定期复查、对症处理后,都能治愈,值得临床推广。Objective To observe the effect of YAG laser nasal lacrimal sac fenestration using endoscopic in treatment of chronic dacryocystitis. Methods Retrospective clinical analysis of 82 clinical data（ 106 eyes）of YAG laser nasal lacrimal sac fenestration using endoscopic in treatment of chronic dacryocystitis from the sep- tember 2002 to december 2009 in E. N. T. department of the second affilitated hospital to Mudanjiang medical university. Results 75 patients of epiphora and pyorrhea symptoms all disappeared after operation, syphilis of lacrimal passages were unobstructed. 5 cases of epiphora recured after 1 month and found obstruction in tears mains, symptoms disappeared after using tears mains YAG laser treatment. The epithelial degree of windowing in other two cases was not ideal. All patients reached the criteria of clinical cure, followed up for 3 to 24 months, there had no recurrence. Conclusion YAG laser nasal lacrimal sac fenestration using endoscopic in treatment of chronic dacryocystitis had positive curative effect and was worthy of promotion, the chronic daeryoeystitis can be cured if recheck regularly after operation and give symptomatic treatment.
