改变婴儿完全性唇腭裂松弛切口的位置对上颌骨发育的近期影响

摘　　要：目的观察单侧完全性唇腭裂早期腭裂手术时不同松弛切口位置对上颌骨发育的影响。方法 60例均在婴儿期一期完成唇腭裂手术患儿分为两组,其中腭裂采用新松弛切口的30例为实验组,随访时平均年龄(8.99±0.79)岁;30例采用传统松弛切口的为对照组,随访时平均年龄(9.11±1.07)岁;另30例无先天性唇腭裂的9岁龄学童作为正常组;三组分别取头颅侧位片与上牙颌模型,进行测量分析。结果两组唇腭裂患儿之间的上颌骨发育无明显差异,与正常组相比较,均有生长抑制。结论婴儿期行唇腭裂手术的完全性唇腭裂患儿在9岁左右上颌骨发育与正常儿童存在明显差异,不同松弛切口位置对上颌骨发育无明显影响。Objective To observe the impact of different location of relaxation incision in early cleft palate repair on maxillary growth in unilateral complete lip and palate cleft patient.Methods 60 patients with one-stage cleft lip and palate repair performed in infancy were divided into two groups according to the location of relaxation incision.In the experimental group,30 patients were undertaken the new relaxation incision in cleft palate repair,their mean age was （8.99±0.79）years in follow-up;another 30 patients with traditional relaxation incision was the control group,The mean age of this group was（9.11±1.07） years old in follow-up;As well as thirty non-congenital cleft lip and palate children with age of 9 years as normal group;The X-ray cephalometry and dentognathic model of three groups children were obtained and comparison.Results The maxillary growth have no significant difference between the two groups children with cleft lip and palate;but contrasting to the normal group,the two groups cleft lip and palate patients all had the growth inhibition.Conclusion The complete cleft lip and palate patient with the infantile cleft lip and palate repair had obvious difference on the maxillary development with the normal children in nine years old,the different location of relaxion incision have no obvious influence on maxillary growth.
