摘 要:目的探讨实时超声造影在乳腺肿瘤鉴别诊断中的价值。方法经超声造影检查并手术或活检组织病理证实的乳腺肿瘤患者57例,63个病灶,其中恶性病灶34个,良性病灶29个。行超声造影检查,以QLAB软件分析病灶内造影剂起始灌注时间、达峰时间、峰值强度、造影增强模式和造影剂廓清模式。结果 85.3%的乳腺恶性病灶表现为非均一性的增强,79.3%的良性病灶主要表现为均质性增强。89.7%的良性病灶造影剂快速消退,88.2%的恶性病灶造影剂缓慢消退,良恶性病灶的消退模式差异有统计学意义(P<0.05)。良恶性病灶起始灌注时间、达峰时间差异无统计学意义(P>0.05),但病灶值强度高于恶性病灶(P<0.05)。结论实时超声造影对乳腺肿瘤的鉴别诊断是有帮助的,其中病灶中造影剂的廓清模式值得关注。Objective To investigate the clinical value of real - time contrast - enhanced uhrasonography in differential diagnosis of breast tumor. Methods Fifty - seven patients with breast tumor were enrolled in this study, and their 63 lesions were confirmed by contrast -enhanced uhrasonography combined with surgery and biopsy tissue pathology, including 34 malignant lesions and 29 benign lesions. Then they were examined by ultrasonography, and the initial time of perfusion (ITP) , time to peak (TI'P) , peak intensity (PI), the enhancement pattern (EP) and the wash out pattern of contrast agent were analyzed by QLAB software. Results 85.3% of breast malignant lesions showed heterogenous enhancement, while the benign lesions showed homogenous enhancement. The persistence time of the contrast agents lasted obviously longer in malignant lesion than benign ones. There were no significant differences in the value of ITP and TFP between the malignant lesions and the benign lesions (P 〉 0.05 ), while the PI of malignant lesions was significantly higher than that of benign lesions ( P 〈 0.05 ). Conclusion Real - time contrast - enhanced ultrasonography is helpful for differential diagnosis of breast tumors, however, the wash out pattern of the contrast agent inside the lesion might be paid more attention.
