摘  要:目的探讨超声检测胎儿大脑中动脉血流峰值流速(MCA-PSV)对产前预测重型α-地中海贫血的价值。方法超声检测399例不同孕龄胎儿MCA-PSV,显著升高(高于同孕龄胎儿中位数的1.5倍)者行脐静脉穿刺及基因诊断。结果 16例胎儿MCA-PSV显著升高,其中重型α-地中海贫血胎儿12例,重型β-地中海贫血1例,胎儿多发畸形并梅毒感染1例,假阳性2例。重度贫血胎儿MCA-PSV较同孕龄正常对照组胎儿MCA-PSV明显增高,两组差异有统计学意义(P<0.001)。以MCA-PSV高于同孕龄胎儿中位数的1.5倍为标准,超声预测重度胎儿贫血的敏感性为100%,假阳性为12.5%。结论超声检测MCA-PSV是产前预测胎儿重型α-地中海贫血的有效方法。Objective To investigate the diagnostic value of middle cerebral artery peak velocity(MCA-PSV) in forecasting severe α-thalassemia in fetuses with ultrasound. Methods MCA-PSV in 399 fetuses was gained. Umbilical vein puncture was performed in 14 cases whose MCA PSV was significantly high. Results Among these 339 feruses there were patients with severe α-thalassemia. MCA-PSV in fetuses with severe α-thalassemia was significantly higher than in normal fetuses. The difference was statistically significant(P〈0. 001). Conclusions It is an effective way to forecast severe α-thalassemia by detecting MCA-PSV with ultrasonography.
