摘 要:目的探讨3.0T磁共振磁敏感加权成像(SWI)及常规MRI序列弥散加权成像、T_1WI、T_2WI对脑微出血的诊断价值。方法经SWI检测存在脑微出血的脑血管病患者21例,通过多序列对比研究,分析脑微出血在各序列中的影像学表现、分布及检出敏感性。结果脑微出血在各序列上均表现为圆形或椭圆形低信号缺失。SWI序列检出脑微出血总数目261个,平均每例患者(12.4±9.4)个,弥散加权成像、T_1WI、T_2WI各序列检出情况依次为96、55、66个,平均每例患者依次为(4.6±4.0)个、(2.6±2.8)个、(3.1±3.1)个。以SWI为金标准,各序列脑微出血检出敏感性依次为36.8%,21.1%和25.3%。SWI序列较弥散加权成像、T_1WI、T_2WI序列脑微出血检出数目明显增多(P<0.01)。SWI序列上脑微出血分布以皮质-皮质下区最多,其次是基底节、丘脑、脑干和小脑区。结论 SWI序列是诊断脑微出血的理想方法。常规序列对脑微出血的诊断有重要提示作用。Objective To evaluate the value of susceptibility weighted imaging(SWI) and other sequences(T1WI,T2WI and DWI) for the diagnosis of cerebral microbleeds(CMBs) at 3.0T MR.Methods Twentyone patients with cerebrovascular diseases were scanned with different MRI sequences which included SWI,T1WI,T2WI and DWI.Utilizing results of SWI as gold standard,the sensitivity for detection of CMBs was compared.Results CMBs in each sequence appeared as round or oval hypointensities.A total of 261(12.4±9.4 per patient) CMBs were detected with SWI,while 96(4.6±4.0) with DWI,55(2.6±2.8) with T1WI and 66(3.1±3.1) with T2WI.When SWI was considered as the gold standard,the detection sensitivity of the other three sequences were 36.8%,21.1% and 25.3%.There was statistically significant difference in the number of CMBs detected by different sequences(P〈0.01).The CMBs most frequently detected by SWI were in cortexsubcortical region,followed by the basal ganglia,thalamus,brain stem and cerebellum area.Conclusions SWI is the ideal method for diagnosis of CMBs while conventional sequence is helpful to detection of CMBs.
