摘　　要：目的探讨十二指肠球部后壁溃疡合并大出血的手术治疗,应用改进术式,缝扎出血点、缝合闭合溃疡处十二指肠肠腔的方法的临床效果。方法回顾、总结我院采用丝线缝扎出血灶止血,于十二指肠后壁穿透性溃疡远端、近溃疡处于粘膜层、粘膜下层作内荷包缝合,闭合十二指肠肠腔的改进术式,治疗32例十二指肠后壁穿透性溃疡并大出血患者的临床资料。结果全组病例疗效满意,无发生损伤胆总管及胰腺、十二指肠瘘、再出血等并发症严重后果。结论应用改进的术式,处理十二指肠球部后壁穿透性溃疡并大出血,是简便、有效、可行的。【Objective】To explore the clinical effect of penetrating ulcer with massive hemorrhage in posterior wall of duodenal ulcer through the modified operation.【Methods】To review and summarize the clinical data of using the modified operation to treat 32 patients with penetrating ulcer in posterior wall of duodenal ulcer.This modified operation is to transfixi the hemorrhagic focus to stop bleeding using silk suture,then make a inner purse string suture at mucosa and submucosa in the remote end of the duodenal ulcer.【Results】The curative effect is satisfactory in all the cases,without many serious complications such as choledochus and pancreas injury,duodenal fistula,rehaemorrhagia etc.【Conclusions】It is simple,effective and feasible to deal with the penetrating ulcer with massive hemorrhage in posterior wall of duodenal ulcer through the modified operation.
