犬同种异体原位肝移植实验研究与临床肝移植1例报告

摘　　要：探讨犬原位肝移植技术和临床肝移植术中及围手术期的处理方法，以及液压式转流泵应用的可 行性。方法：在滚压式转流泵进行静脉-静脉转流的情况下，进行１５例犬同种异体原位肝移植实验研究，并为１例 原发性肝癌患者施行同种异体原位肝移植，对术中及围手术期发生的各种问题进行研究和处理。结果：滚压式转 流泵转流效果良好；１５只犬肝移植存活９只；临床肝移植患者存活３．５个月，死于肺部感染，肿瘤无复发。结论：大 动物的肝移植实验研究是临床肝移植的必要技术准备；在肝移植中用液压式转流泵进行静脉-静脉转流安全可行； 肝移植可用于肝癌的治疗。To investigate the operative procedures and perioprative managements of orthogopic liver transplantation and feasibility of roller pump used in the course of operation.Methods:The roller pump was used during both experimental and clinical studies.Anexperimental study on allograft orthotopic liver transplantation was performed in 15 dogs.After getting success ,a clinical allograft orthotopic liver transplantation was performed in a patient with hepatocellular carcinoma.The methods of perioperative management were studied clinically.Results:Veno-venous bypass showed successfully both in experimental and clinical studies.9dogs were surival in the experimental studies;as for the clinical liver transplantation,the patient survied for 3.5 months and died of pneumonia without tumor recurrence.Conclusion:Experimental study of liver transplantation in large animals is necessary technical training. Veno-venous bypass with rollerpump is safe for the operation of liver transplantation.Orthotopic liver transplantation is and effective method for the treatment of primary liver cancer.
