先天性巨结肠症的外科治疗体会

摘　　要：目的：总结17例先天性巨结肠症患儿的手术治疗经验,分析、探讨根治先天性巨结肠症的外科治疗方法及疗效。方法：采用结肠切除、直肠后结肠拖出术（Duhamel手术）治疗5例和经肛门改良Soave术治疗12例。其中,2个月～2岁4例,2～10岁12例,10～18岁1例。结果：17例病例均行手术治疗,无死亡病例,无肠梗阻、肛门狭窄等术后并发症发生。随访6个月-9年,所有患儿排便正常,无大便失禁或污粪。结论：外科手术是治疗先天性巨结肠症唯一有效的治疗方法,强调术前充分的肠道准备及合理的手术方法是治疗成功的关键。Objective：To summary and analyze the experience of surgical treatment on 17 cases of congenital macrosis colon.Methods：Taking the colon resection and Duhamel operations to treat 5 cases and modified Soave’s operation to treat 12 cases.Among them,4 cases were 2 months to 2 years,12 were 2 to 12 years,one was 10 to 18 years.Results：17 cases were received the operation.There were no dead case,and no intestinal obstruction,stricture of anus complications.After following up 6 months to 9 years,the defecation of all the patients was normal and no fecal incontinence.Conclusion：Surgical treatment is the only one therapeutic way of treatment on congenital macrosis colon.
