鼻息肉圈套器治疗咽部乳头状瘤45例疗效观察

摘　　要：目的探讨一种更好的治疗咽部乳头状瘤的方法 ,分析使用鼻息肉圈套器治疗咽部乳头状瘤的效果。方法咽部表面黏膜局部麻醉下对45例患者用筛窦黏膜钳钳夹咽部乳头状瘤并稍用力牵引,用鼻息肉圈套器套入瘤体根部,收紧圈套器摘除瘤体,创面可用干棉球稍加压迫片刻以止血。手术后随访6个月观察疗效。结果本组45例患者使用鼻息肉圈套器治疗咽部乳头状瘤均一次性治愈,手术时间短,2～3s即可切除咽部乳头状瘤,术中出血少,约0.1～0.5ml,术后5d创面伪膜开始脱落,17例患者7d左右创面基本愈合,28例患者10d创面基本愈合。手术后1,3,6个月后复诊,未见一例复发。结论使用鼻息肉圈套器治疗咽部乳头状瘤安全有效、操作方便、术后反应轻、愈合快,值得临床推广。Objective To evaluate the curative effects of polyp snare on pharyngeal papillomas,and explore a preferable therapeutic method for pharyngeal papillomas.Methods With pharyngeal portion mucosa surface anesthesia,the pharyngeal papillomas were clamped by using ethmoid forceps and drawn slightly,than hitched by polyp snare near neoplastic root;By tightening the polyp snare,the neoplasma in 45 patients were excised.The wound surface was pressed to stop bleeding by using dry cotton ball.The curative effect was observed within 6 months after the operation.Results All the pharyngeal papillomas of 45 patients were cured by using polyp snare one time.The process of operation was short（mean 2 to 3 seconds）,blood loss was approximately 0.1 to 0.5 ml;the pseudomembranes of the wound surface began to desquamate 5 days after the operation.The raw surface cicatrized within 7 days in 17 cases,and within 10 days in other 28 cases.No recurrent case was found in the follow up of 1,3 and 6 months.Conclusion Polyp snare,with a light postoperative adverse reaction and short healing time,is safe and easy methods for pharyngeal papillomas.It is worthy to be recommended in the clinic.
