摘　　要：目的观察阿尔茨海默病和正常老龄脊髓组织标本α-synuclein相关病理改变特点。方法采集3例阿尔茨海默病(2例经典阿尔茨海默病、1例伴路易小体阿尔茨海默病)和7例老龄非痴呆性尸体的脊髓组织标本,常规HE染色和免疫组织化学染色观察α-synuclein在脊髓组织中的表达情况。结果在伴路易小体阿尔茨海默病患者的延髓迷走神经背核、中脑红核以及脊髓组织中同时发现α-synuclein相关病理改变,胸髓侧角中间外侧柱和骶髓灰质腹侧前外侧部组织中可见少至中量Lewy小体和散在分布的Lewy轴索样结构。结论伴路易小体阿尔茨海默病患者脊髓内有α-synuclein表达阳性的Lewy小体或Lewy轴索样结构存在,且与脑组织α-synuclein相关病理改变存在明显相关性。Objective To investigate the characteristics of α-synueleinopathy in the spinal cord of patients with Alzheimer＇s disease （AD）. Methods HE staining and immunohistochemistry staining were used to determine the expression of α-synuelein in the spinal cord （T2, T3, T10, L2 and S2 segments）of 3 patients with AD, one of them was an Alzheimer＇s disease with Lewy bodies （ADLB） patient. Seven age- matched non-dementia and non-neurological diseases patients served as controls. Results α-synuclein positive structures were found in dorsal nucleus of vagus nerve （DNVN） of medulla oblongata, red nucleus （RN） of midbrain and spinal cord of ADLB patient. Lewy body （LB） and dispersed Lewy neurites （LN） were encountered in the intermediolateral column of lateral horn in thoracic cord and the anterolateral of ventral gray in sacral cord of the former cases. Conclusion Lewy body and Lewy neurites with positive expression of α-synuclein can be seen in spinal cord of ADLB patients, and it seems to be positive correlated with α-synucleinopathy in brains.
