帕米膦酸二钠治疗恶性肿瘤骨转移疼痛

摘　　要：目的：观察国产帕米膦酸二钠（ＡＰＤ）对恶性肿瘤溶骨转移疼痛的止痛作用和不良反应，探索理想用药方法。方法：３３例患者接受ＡＰＤ治疗，分为单次给药组（１２例）：ＡＰＤ６０～１２０ｍｇ／次（平均８５ｍｇ）一次给药，分次给药组（２１例）：ＡＰＤ３０ｍｇ／次，ｑｄ，连用３ｄ。结果：对骨转移疼痛的总有效率和显效率分别为７５．８％和２４．２％。单次和分次给药组止痛有效率分别为７５．０％和７６．２％（Ｐ＞０．０５），显效率分别为５８．３％和４．８％（Ｐ＜０．０１）。对活动能力改差善的总有效率和显效率分别为５８．３％和２９．２％。单次和分次给药组的有效率分别为８１．８％和３８．５％（Ｐ＜０．０５），显效率分别为５４．５％和７．７％（Ｐ＜０．０５）。大部分病例３ｄ起效，维持２８ｄ。不良反应主要为发热（２１．２％）。单次给药组有２例出现一过性无症状低钙血症，均可自行缓解。结论：国产ＡＰＤ治疗恶性肿瘤骨转移疼痛疗效肯定，相同剂量下单次较分次给药对骨转移疼痛减轻程度更佳。不良反应轻微，短暂。OBJECTIVE :To observe the analgesic effect of pamidronate(APD)and its adverse reactions in treatment of metastatic bone pain. METHODSWT]:The study was conducted in 33 patients with metastatic bone pain.They were divided to receive single dose of 60120 mg APD(n=12)or once daily dose of 30 mg APD for 3 days(n=21). RESULTS :The total response rate and total effectual rate in relief of metastatic bone pain were 75.8% and 24.2% respectively.the corresponding data for improvement in daily living(ADL)were 58.3% and 29.2% respectively.The onset of analgesic action in most of patients was 3 d and maintained for 28 d.The most common sideeffects was transient seltlimiting pyrexia and 2 cases of transient asymptomatic hypocalcemia occurred in single dose group. CONCLUSION :APD is effective in treatment of metastatic bone pain.The painrelief effect in single dose group is more pronounced than that of other group on the basis of equal dose.
