摘 要:【目的】观察腹腔内压力(1AP)增高对心、肺、肾功能的影响。【方法】对72例腹腔间隙综合征(ACS)患者,每日四次在固定时间检测患者UBP,记录平均动脉压(MAP)、中心静脉压(CVP)、心率(HR)、呼吸频率(RR)、动脉氧分压(PaO2)、动脉二氧化碳分压(PaCO2),同时检测血清尿素氮(BUN)、肌酐(Cr),记录每日尿量。【结果】随着IAP升高,CVP、HR增加,MAP下降;RR增快,PaCO2潴留,而PaO2降低;尿量逐渐减少,血清BUN和Cr上升(P〈0.05)。IAP上升与尿量呈负相关(r=-0.193,P〈0.05);与血清Cr呈正相关(r=0.162,P〈0.05),.但与MAP、CVP、HR、RR、PaO2、PaCO2、BUN无相关性。【结论】IAP可以导致循环、呼吸及肾脏功能的损害,IAP的上升与少尿、血清Cr上升密切相关。[Objective] To observe the effect of the increasing of intraabdominal pressure(IAP) on the circula tion, lung and kidney function. [Methods] Seventy two patients with abdominal compartment syndrome(ACS) were enrolled in this study. The IAP was measured four times a day at a certain time. Mean arterial pressure (MAP), central venous pressure(CVP), heart rate(HR), respiratory rate(RR), PaO2 and PaCO2 were recorded. Serum BUN and Cr were detected. Urine volume per day was also recorded. [Results] As the IPA increased, CVP and HR increased and MAP decreased. RR increased and PaCO2 retained, but PaO2 decreased. Urine volume decreased gradually. Serum BUN and Cr increased( P 〈0.05). The increasing of IAP was negatively correlated wit h urine volume (r=-0. 193, P 〈0.05), and positively correlated with serum Cr(r=0. 162, P 〈0.05), but had no correlation with MAP, CVP, HR, MAP, RR, PaO2, PaCO2 and BUN. [Conclusion] IAP can lead the damage of circulation, respiratory and renal function. The increasing of IAP is closely correlated with the rising of oliguria and serum Cr.
