胃窗超声造影加组织谐波成像技术在早期胃癌诊断中的应用

摘　　要：目的研究胃窗超声造影(OUCA)和组织谐波成像(NTHI)技术在早期胃癌(EGC)诊断中的临床应用价值。方法对25例EGC患者采用基波成像(FI)和NTHI成像技术,观察二者对小病灶胃癌显像的差别以及NTHI对图像质量的改善程度,综合分析超声检查在EGC诊断中的临床意义。结果超声对病灶的检出率为76%,对病灶浸润范围测值的准确性为84.2%,对病灶浸润深度判断的符合率为52.6%。FI图像质量与病灶部位、患者体型及胃充盈情况密切相关,各种原因FI不满意的患者在THI状态下得到了明显改善,平均改善率为88.2%,二者差异有统计学意义(P<0.05)。结论THI技术能显著改善图像质量,增强病灶与周围组织间的对比,超声成像对胃壁异常增厚改变的敏感性较高,可对EGC的发现提供重要参考信息,有助于提高超声对EGC的检出率;对EGC诊断具有重要价值。Objective To investigate clinical value of oral ultrasonic contrast agent（OUCA）and tissue hanrmonic-imaging（NTHI） technique in diagnosis of early gastric carcinoma（EGC）.Methods Fundamental wave image（FI） and NTHI were performed in 25 early gastric carcinoma cases and the improvement of image quality were observed by NTHI.Clinical value of ultrasonography was evaluated in the diagnosis of early gastric carcinoma.Result The lesions found by ultrasonography were 100%,while the infiltrative range was 84.2% and the diagnostic coincidence rate of the eroded depth was 52.6%.The image quality by FI was significantly related to the lesion locations,patients somatotype and situation of the stomach.The image quality of any cases who were not suitable to FI were obviously improved by NTHI.Improvement rate averaged 88.2%,and the image quality showed significant difference between FI and NTHI（P〈0.05）.Conclusion NTHI can effectively improve image quality and enhance the contrast between focus of infection and the surrounding tissue.Sensitivity of ultrasonic imaging for diagnosing abnormal change of partial stomach is high and it can provide important message for discovering of EGC.It also enhances the finding rate of early gastric carcinoma and it is very useful and practical for the quantitative diagnosis of early gastric carcinoma.
