慢性非器质性头痛与焦虑抑郁情绪的关系

摘　　要：目的探讨就诊于神经内科门诊的慢性非器质性头痛患者中焦虑抑郁的发生率及其特点,为临床治疗提供依据。方法对2008年1月-2009年6月来我院神经内科就诊的355例慢性非器质性头痛患者采用汉密顿抑郁和焦虑量表进行测定,并同时记录性别、年龄、病程、文化程度等。结果 45.63%的头痛患者伴发情绪障碍,单纯焦虑占11.0%,单纯抑郁占9.3%,焦虑抑郁并存者占25.4%。有焦虑症状的头痛患者出现抑郁症状显著高于无焦虑症状患者（P〈0.01）;反之亦然。头痛病程越长,焦虑抑郁发生率越高（P〈0.01）。女性头痛患者伴随焦虑抑郁者显著高于男性。焦虑抑郁症状在不同的文化程度和职业之间差异无统计学意义。结论慢性非器质性头痛常伴有焦虑抑郁情绪,病程越长,焦虑抑郁越严重,特别是女性和年龄〈40岁者。对头痛患者存在的焦虑抑郁需要及早发现、及早干预。Objective To investigate the prevalence and characteristics of anxious/depressive disorder with chronic non-organic headache,in purpose of effective treatment in clinical practice.Methods 355 outpatients with chronic non-organic headache were tested with HAMD and HAMA from January 2008 to June 2009.Additional information as gender,age,course of disease,education level were analyzed.Results 45.63%of the patients with headaches（62/355）associated with mood disorder.The incidence of anxiety,depression and co-existence of anxiety and depression accounted for 11.0%,9.3% and 25.4% respectively.Depressive symptoms appearing in the patients with headache accompanied by anxiety symptoms were significantly higher than those in the patients without anxiety symptoms（P〈0.01）;and vice versa.The longer duration of headache,anxiety and depression more serious（P〈0.01）.Female patients with headache showed higher anxiety and depression score than men significantly.Anxiety and depression symptoms was no significant difference between the different educational level or occupation.Conclusion Chronic non-organic headaches often accompanied by anxiety and depression,the longer the duration,anxiety and depression more serious,especially women and younger than 40 years of age.Early detection anxiety and depression in patients with chronic headache is necessary.
