外周血肿瘤标志物与肠癌预后关系的临床研究

摘　　要：目的检测肠癌患者外周血中生存素(survivin)基质金属蛋白酶2(MMP-2)及其抑制物(TIMP-2)的基因表达,并探讨其在肿瘤诊断、转移及预后判断中的作用。方法对101例肠癌、68例癌前病变及50例正常对照者,采用实时定量反转录聚合酶链反应(RT-PCR)方法检测外周血中survivin mRNA、MMP-2 mRNA和TIMP-2 mRNA的表达水平。并分析其与临床病理因素之间的关系。结果 Survivin mRNA的转录在正常对照组未检测到,良性病变者表达率为7.4%,肠癌组表达率为72.3%,其阳性率与疾病分期及淋巴转移等相关。肠癌患者MMP-2mRNA表达均高于正常对照和良性病变组(P<0.005);MMP-2 mRNA和TIMP-2 mRNA/MMP-2 mRNA比值与肿瘤分期及转移相关(P<0.01);Sur-vivin mRNA高转录者5年生存率显著低于低转录者(P<0.01)。结论 Survivin mRNA、MMP-2 mRNA的高表达、TIMP-2 mRNA/MMP-2 mRNA比值与肠癌的分期、转移密切相关,是肠癌转移和评估预后的良好指标,联合检测有更高的判断价值。Objective To detect the expression of survivin, matrix metalloproteinase-2 （MMP-2） and tissue inhibitors-2 （TIMP-2） gene in peripheral blood of patients with colorectal carcinoma and to discuss the relationship between them and carcinoma. Methods Levels of survivin mRNA, MMP 2 mRNA and TIMP-2 mRNA expression from 101 patients with coloreetal carcinoma, 68 patients with non cancerous lesions and 50 normal control were detected by reverse transcription real time polymerase chain reaction （RT-PCR） and the data was analyzed clinical data. Results Survivin mRNA expression rate in patients with non-cancerous lesions was 7.4%. In patients with colorectal carcinoma, the expression rates suvivin mRNA were 72.3% in colorectal carcinoma and the results were statislically associated with lymph node involvement and staging of the disease. MMP-2 mRNA expression in peripheral blood was siginifcant elevated in patients with colorectal carcinoma compared with that of normal control and non-cancerous lesions, while MMP-2 mRNA. MMP-2 mRNA / TIMP-2 mRNA associated with staging and metastasis. Five-year survival rate was lower in patients with higher survivin mRNA expression. Conclusions High expression of suvivin mRNA, TIMP-2/MMP 2 are obviously correlated with both stage and metastasis in colorectal carcinoma. They may be also the important markers of metastasis and prognosis in colorectal carcinoma. The combination of detecting them may be a more useful tool.
