妊娠期高血压疾病剖宫产术中联合用药预防产后出血的效果观察

摘　　要：目的观察妊娠期高血压疾病剖宫产应用卡孕栓联合缩宫素预防产后出血的效果。方法将2008年7月—2009年10月在本院住院患有妊娠期高血压疾病且行剖宫产结束妊娠的单胎产妇204例用简单随机抽样法分成实验组和对照组。实验组舌下含服卡孕栓联合用缩宫素,对照组只应用缩宫素。用药后测量产后出血量并观察用药前后血压和脉搏的变化及用药后有无消化道症状。结果术中及术后2 h出血量实验组均明显少于对照组（P均〈0.01）,产后2-24 h出血量、用药前后血压和脉搏变化2组比较均无显著性差异（P均〉0.05）,2组用药后无明显消化道症状。结论对于患妊娠期高血压疾病的剖宫产产妇预防宫缩乏力性产后出血,选用卡孕栓联合缩宫素效果最佳,是预防产后出血的有效方法。Objective It is to observe the effect of Methyl Carboprost-NEGPF united with oxytocin preventing beforehand postpartum hemorrhage during cesarean section of patients with hypertension disease in gestation period.Methods 204 patients with hypertension disease in gestation period in our hospital from July 2008 to October 2009 who were ended pregnancy with cesarean section were randomly divided into experimental group and control group.The experimental group was given Methyl Carboprost-NEGPF buccal sublingually united with oxytocin.The control group was only given oxytocin.The volume of postpartum hemorrhage was measured after medication.The changes of blood pressure and pulse were observed before and after medication.With or without gastrointestinal symptoms was observed after medication.Results The volumes of hemorrhage in experimental group during operation and after operation 2 h were both obviously fewer than that in control group（both P〈0.01）.There were all no significant differences on the volumes of hemorrhage after operation 2 to 24 h and the changes of blood pressure and pulse before and after medication（all P〉0.05）.The gravida in the two groups had both no obvious gastrointestinal symptoms after medication.Conclusion To prevent beforehand postpartum hemorrhage induced by uterine inertia during cesarean section of patients with hypertension disease in gestation period,the effect of Methyl Carboprost-NEGPF united with oxytocin is the best,which is an effective method for preventing beforehand postpartum hemorrhage.
