CDK4、PTEN蛋白与子宫内膜癌浸润转移的关系

摘　　要：目的通过研究子宫内膜癌组织中细胞周期素依赖性蛋白激酶(CDK4)、10号染色体丢失的与张力蛋白同源的磷酸酶基因(PTEN)蛋白的表达,探讨二者与子宫内膜癌浸润转移的关系,为临床上寻找判断子宫内膜癌浸润转移情况的生物学标志物提供理论依据。方法采用免疫组化S-P法测定CDK4及PTEN蛋白在50例子宫内膜癌患者手术标本、10例正常子宫内膜、12例不典型增生子宫内膜组织中的表达。结果 1.CDK4在正常子宫内膜组织中表达的阳性率为30.0%,在不典型增生子宫内膜和子宫内膜癌中表达的阳性率分别为66.7%、72.0%。后两者与正常子宫内膜相比CDK4蛋白的表达有明显差异(P<0.05)。2.CDK4蛋白表达阳性率随临床分期升高而有增高的趋势,Ⅱ期及Ⅲ期的阳性表达率与Ⅰ期相比有显著性差异(P<0.05)。3.在不同组织学分级中,CDK4在G2及G3中的阳性表达率与G1比较,差异有显著性(P<0.05)。4.CDK4蛋白在肌层浸润>1/2或有淋巴结转移组中阳性表达率为88.0%。明显高于无肌层浸润或浸润<1/2组(P<0.05)。5.术后复发者CDK4蛋白阳性表达率(93.3%)明显高于未复发者(57.6%),两者相比差异具有显著性(P<0.05)。6.PTEN蛋白在子宫内膜癌中的表达阳性率为56.0%。明显低于正常子宫内膜组织中的表达(100.0%)以及不典型增生子宫内膜中的表达(91.0%)(P<0.05)。PTEN蛋白在无肌层浸润或肌层浸润<1/2组中阳性表达率为76.0%。肌层浸润>1/2或有淋巴结转移者为36.0%,与子宫内膜癌及肌层浸润程度显著相关(P<0.05),但与临床分期、组织学分级、有无复发均无关(P>0.05)。结论 PTEN蛋白在子宫内膜癌中表达下降或缺失及CDK4在子宫内膜癌组织中的过度表达与子宫内膜癌的浸润转移密切相关,检测子宫内膜癌组织中CDK4及PTEN的表达可作为判断子宫内膜癌浸润转移情况的有用标志物。Objective To study the relationship between cyclin dependent kinase-4（CDK4）,PTEN protein expression and carcinogensis,progression and recurrence of endometrial carcinoma.Methods A specifically and sensitively immunohistochemical S-P assay was used to detect the expression of CDK4 and PTEN in paraffin-embedded section of 50 cases of endometrial carcinoma.12 atypical hyperplasia of endometrium and 10 normal endometrium.Results The positive rate of CDK4 was 30.0% in normal endometrium.The CDK4 positive rate was higher in atypical hyperplasia of endometrium than in normal endometrium（P0.05）.The CDK4 positive rate in cancer was significantly higher than that in normal endometrium（P0.05）.There was a different between stage 2 and 3 with stage 1（P0.05）.The overexpression of CDK4 in moderatelypoorly differentiated（G1-G3） carcinoma was higher than that of G1 carcinoma（P0.05）.The positive rate of CDK4 in 1/2 muscle invasion cancer were significantly higher than those in 1/2 muscle invasion（P0.05）.In 41 case followed up,the positive expression and overexpression rates of CDK4 were significantly higher in 15 cases with recurrence than 26 cases without recurrence（P0.05）.The positive rate of PTEN was 100.0% in normal endometrium.The positive rate of PTEN was 56.0% in atypical hyperplasia of endometrium.The PTEN positive rate was higher in atypical hyperplasia of endometrium or normal endometrium than in endometrial carcinoma（P0.05）.The expression of PTEN correlated with muscles invasion（P0.05）,but not with clinical stage and histological grade or recurrence.Conclusion Our results suggest that the aberrant expression of CDK4 and PTEN are closely related to the occurrence,development and recurrence of endometrial carcinoma,and CDK4 and PTEN may be used as a predicting marker of the prognosis of endometrial carcinoma.
