化疗栓塞后肝细胞癌细胞凋亡的研究

摘　　要：目的研究肝细胞癌患者经动脉化疗栓塞（ｔｒａｎｓａｒｔｅｒｉａｌｃｈｅｍｏｅｍｂｏｌｉｚａｔｉｏｎ，ＴＡＣＥ）治疗后肿瘤细胞凋亡的情况。方法用光学显微镜（简称光镜）和流式细胞仪对１１７例手术切除的肝细胞癌标本进行凋亡检测；其中单纯手术５８例，４种介入治疗后二期手术切除５９例。结果流式细胞仪检测肿瘤细胞的凋亡率，单纯手术组为１．７７％，行介入治疗的Ａ、Ｂ、Ｃ、Ｄ４个治疗组分别为６．１１％、８．７１％、１３．２９％、１５．７３％，经秩和检验，除Ｃ、Ｄ组间比较Ｐ＞０．０５，其余两两比较Ｐ值均＜０．０５。末次ＴＡＣＥ与手术切除间期＜１个月、１～２个月、２～３个月、≥３个月，其平均凋亡率分别为１２．７％、１３．５％、８．５％、５．２％，与对照组比较Ｐ值均＜０．０５。光镜观察凋亡指数Ｄ组高于Ｃ组、高于Ｂ组、高于Ａ组、高于单纯手术组。结论细胞凋亡是ＴＡＣＥ引起肝癌细胞死亡的另一重要机制，且作用持续时间长。碘油、乙醇、明胶海绵联合栓塞优于单一材料栓塞。Objective To study apoptosis of hepatocellular carcinoma (HCC) cells after transcatheter arterial chemoembolization. Methods One hundred and seventeen histologically verified HCC specimens were studied. The patients were treated with surgical resection alone (58 cases), second stage surgical resection after four kinds of interventional treatment(59 cases). Microscope and flow cytometry were used to observe the apoptosis. Results Quantification of apoptosis using flow cytometry indicated that the apoptosis index(AI) of HCC cells accounted for 1.77% in first stage resection group, 6.11% in chemotherapy alone,8.71% in chemotherapy combined with iodized oil, 13.29% in chemotherapy combined with iodized oil and spongia gelatini absorbens(Sga), 15.73% in chemotherapy combined with iodized oil, ethanol and Sga. Except for the latter 2 groups with P >0.05, all other comparisons showed P <0.05.H.E staining slides showed typical morphological appearence of apoptosis. AI of HCC cells <1,1-2,2-3 and ≥3 months after TACE was 12.7%、13.5%、 8.5%、 5.2% respectively. Conclusions Appoptosis is another important mechanism leading to cell death in HCC after TACE. Combined embolization of iodized oil、ethanol、and Sga is superior to that using single material.
