摘 要:目的通过研究兔颈动脉变异情况,探讨建立良好的适应临床影像学研究需要的脑缺血动物模型。方法46只新西兰兔随机分为缺血组(40只)和对照组(6只)。缺血组采用颈外动脉逆行置管,暂时夹闭颈总动脉,由导管内注入自体血凝块,建立脑缺血模型。对照组栓塞前手术过程与缺血组相同。缺血组在栓塞后不同时限行MRIT2WI检查及病理检查。结果46只兔颈动脉变异共分为4型:Ⅰ型,单纯颈内动脉型(26条,占57%);Ⅱ型,颈内动脉近端发出枕动脉分支型(11条,占24%);Ⅲ型,颈内动脉远端发出枕动脉分支型(3条,占6%);Ⅳ型,颈内动脉发出耳动脉分支型(6条,占13%)。根据24小时MRIT2WI所确定的梗死灶将30只存活兔分为3型:局灶型(44%),基底节和(或)小叶皮层型(33%),以及半球型(23%)。结论Ⅰ、Ⅱ、Ⅳ型适宜制作兔脑缺血模型,其中Ⅱ型需暂时夹闭枕动脉,Ⅲ型不适宜用于制作脑缺血模型。这种改进的脑缺血模型为临床影像学研究需要提供了实验基础。Objective To investigate the carotid artery variations of rabbit, we in order to establish a cerebral ischemic animal model which can adapt to the need of clinical study. Methods Forty six cases of rabbits are arranged into 2 groups: ischemic group (40 cases) and control group(6 cases). To set up the cerebral ischemic model, we retrogradely catheterized the external carotid artery, temporarily clipped the common carotid artery, then injected the autocoagnlated clot. The operation process of control group was the same as ischemic group before injecting the clot. MR T 2 weighted images and pathological histology examinations were done in ischemic group at different time span. Results Four types were classified according to variations: Type Ⅰ, simple internal carotid artery (57%); TypeⅡ, occipital artery arising from the proximal internal carotid artery (24%); TypeⅢ, occipital artery arising from the distal internal carotid artery(6%); Type Ⅳ, auricular artery arising from internal carotid artery(13%). Three types of ischemia were classified on the basis of the manifestations on MR T 2 weighted images: Focal type(44%); Basal ganlia and/or lobular cortex type(33%) and hemispheric type (23%). Conclusion Type Ⅰ, Ⅱ and type Ⅳ can be used for cerebral ischemic model whereas for type Ⅱ, temporary occlusion of the occipital artery was needed. Type Ⅲ can't be used as cerebral ischemic model. It is was concluded that the improved model provides a good experimental basis for the study of human ischemic stroke.
