摘 要:目的探讨重组人促红细胞生成素(rhEPO)对大鼠急性心肌梗死(AMI)后24h血流动力学和左心室梗死面积的影响。方法 24只雄性SD大鼠随机分为假手术组、AMI组和EPO组,结扎左冠状动脉前降支建大鼠AMI模型,EPO组同时给予rhEPO5000U/kg腹腔内注射;24h后记录大鼠血流动力学参数并计算左心室梗死区与左心室质量比值。结果 AMI组各项血流动力学参数均明显下降;EPO组血流动力学参数均明显改善(P<0.05,P<0.01),尤其反映心肌舒张功能的指标改善更显著(P<0.01);EPO明显降低左心室梗死区与左心室质量比值(P<0.05)。结论 EPO急性干预缩小大鼠AMI后的梗死面积并明显改善其血流动力学参数,尤其对舒张功能改善更为明显。Objective To investigate the effect of erythropoietin (EPO) on the 24 hours hemodynamic status and infarct area of the left ventricle in rats after acute myocardial infarction (AMI).Methods 24 male SD rats were recruited and divided into 3 groups at random:sham-operated,AMI and EPO groups.AMI rat model was created by ligating left anterior descending coronary artery.The rats in EPO group were treated at same time with intraperitoneal injection of recombinant human EPO (rhEPO,5 000 U/kg).The hemodynamic parameters including blood pressure (BP),left ventricular systolic pressure (LVSP),maximal rate of increase of ventricular pressure (dp/dtmax),left ventricular diastolic pressure (LVDP),maximal rate of decrease of ventricular pressure (dp/dtmin) and left ventricular end diastolic pressure (LVEDP) in rats 24 hours after operation were determined.The size of infarct myocardial tissue and the ratio of the weight of infarct myocardial tissue as well as that of the whole left ventricle were measured.Results Compared with sham-operated group,all hemodynamic parameters of rats declined remarkably in AMI group.Compared with AMI group,all hemodynamic parameters of rats in EPO group were improved in different degrees:LVSP and dp/dtmax related to cardiac systolic function were slightly improved but the differences were not significant (P0.05);LVDP,LVEDP as well as dp/dtmin associated with cardiac diastolic function were remarkably improved (P0.05 or P0.01);mean arterial pressure (MAP) showed no difference (P0.05).The area of infarct myocardial tissue in the left ventricle and the ratio of the weight of infarct myocardial tissue as well as that of the whole left ventricle in EPO group were significantly reduced compared to those in AMI group (P0.05).Conclusions Application of EPO can ameliorate the hemodynamic parameters in rats with AMI,especially improve the cardiac diastolic function.Treatment of EPO can reduce the myocardial infarct area of AMI rats.
