11例血友病性关节炎行全膝关节置换术的围手术期护理

摘　　要：总结了11例甲型血友病性关节炎患者(14个膝关节)行全膝关节置换术的围手术期护理经验。对患者实施围手术期的全程凝血因子Ⅷ监测和替代治疗;注意选择安全的备皮方法和注射方式;术后密切观察、及时处理切口渗血、关节腔积血,保持切口引流管通畅,对术后8h内引流血液进行自体回输;在凝血因子Ⅷ输入后30min行功能锻炼。患者均安全度过围手术期。This paper summarizes the experience of perioperative nursing of 11 patients （14 knees） with hemophilia A arthritis undergoing total knee replacement.The whole process of perioperative monitoring and replacement therapy of blood clot Ⅷ were implemented.In addition,nursing care focused on choosing safe methods of skin preparation and injection,close observation of incision oozing of blood and articular cavity hemorrhage,keeping unobstructed of incision drainage tube,auto-transfusion of losing drainage blood within 8 hours after the surgery,and soft functional training after 30 minutes of blood clot Ⅷ transfusion.All the patients safely got through the perioperative period with satisfying recovery.
