不同屈光矫治后近视眼调节功能的研究

摘　　要：目的:探讨屈光矫治方式对近视眼患者眼调节功能的影响。方法:近视眼患者146例,根据屈光矫治方式分为框架眼镜、角膜接触镜和屈光手术3组,另取无近视的健康对照组50例。分别检查单眼调节幅度、调节灵敏度和调节滞后量。结果:单眼调节幅度以框架眼镜组最低(P<0.05),而角膜接触镜组、LASIK组和健康对照组3组间没有差异(P>0.05);调节灵敏度3种矫治方式后均和健康对照组一致(P>0.05);调节滞后量以框架眼镜组最低(P<0.05),而角膜接触镜组、LASIK组和健康对照组3组间没有差异(P>0.05)。相关分析显示调节幅度与眼屈光度有相关性(r=0.714,P=0.011);调节灵敏度与屈光不正无明显相关性(r=-0.187,P=0.304);而调节滞后量与眼屈光度有相关性(r=-0.677,P=0.002)。按照近视程度分层分析显示3个调节指标和分层前趋势基本一致。结论:框架眼镜对近视眼患者的眼调节功能存在一定影响,表现为单眼调节幅度和调节滞后量的改变。而角膜接触镜和LASIK屈光矫治后的眼调节功能基本和正常相当。Objective：To investigate the influence of eye adjustment function for different refractive correction of in patients with myopia. Methods：146 patients with myopia were divided into frame glasses group,contact lenses group and refractive surgery group,and the other 50 person were included as healthy control group.Amplitude of monocular accommodation were examined,as well as adjust sensitivity and adjust lag.Results：The amplitude of monocular accommodation in frame glasses group were lowest than that of the other three groups（P0.05）,while in the contact lens group,LASIK group and the healthy control group,there were no significant difference （P0.05）;The adjust sensitivity were same between three groups and the healthy control group（P0.05）;The adjust lag in frame glasses group were lowest than that of the other three groups（P0.05）,while there were no significant difference among the other three groups. Correlation analysis showed that amplitude of monocular accommodation has correlation with refractive degree eye（r=0.714,P=0.011）; adjust sensitivity has no correlation with refractive degree（r=-0.187,P=0.304）;while the adjust lag has correlation with eye refractive degree（r=-0.677,P=0.002）.Accordance with the stratified analysis of degree of myopia,the trends of three adjustment indicators were mostly the same as before layering.Conclusion：The spectacle has some impact for eye adjustment function of myopia,which showed indicate the change of the amplitude of monocular accommodation and adjust lag.The eye adjustment function in contact lens and LASIK is same compared by normal eyes.
