摘 要:目的利用肠易激综合征大鼠模型,观察电针刺激足三里对IBS大鼠慢性内脏高敏感性的影响,为临床提供治疗的科学依据,并研究内源性阿片肽系统是否参与构成电针导致IBS大鼠内脏感觉降低的机制。方法采用新生幼鼠制作IBS慢性内脏高敏感性模型,观察大鼠腹壁肌电(EMG)变化,评价电针刺激足三里对IBS大鼠慢性内脏高敏感性的治疗效应;采用RT-PCR半定量检测大鼠丘脑及肠道组织内源性阿片肽系统μ、κ受体基因表达的变化。结果成年IBS大鼠EMG明显增加,电针刺激足三里后,IBS大鼠EMG明显减少。成年IBS大鼠肠道及丘脑内κ、μ受体基因表达无明显变化;电针刺激足三里后,IBS大鼠丘脑内μ受体表达明显增加,而丘脑内κ受体及肠道内κ、μ受体无明显变化。结论电针刺激足三里可以降低IBS大鼠慢性内脏高敏感性,其功能可能与内源性阿片肽、丘脑内阿片μ受体上调有关,丘脑内κ受体在其中的作用还有待于进一步研究。Objective To find out the therapeutic effect of electroacupuncture(EA) at acupoint ST-36(Zusanli) on chronic visceral hypersensitivity in IBS model of the rats,and to investigate whether the endogenous opioid system participated in the mechanism that EA at acupoint ST-36 depressed visceral hypersensitivity.Methods IBS model was developed in the rat from neonatal age.Behavioral test of abdominal withdraw electrophysiological recording of abdominal external oblique abdominis electromyogram(EMG) was employed in order to find out the effects EA at acupoint ST-36 on chronic visceral hypersensitivity.Once more,we employed RT-PCR to compare the changes of the μ and κ opioid receptor gene expression in colon rectum and thalamencephalon among groups.Results There was a significant increase of abdominal electrical activity in IBS model rats.After EA treatment,the abdominal electrical activity in IBS model rats was significantly lower than before EA.There was no significant change of κ opioid receptor or μ opioid receptor gene expression not only in colon rectum but also in thalamencephal of IBS model rats.After EA treatment,there was a significant ascending of μ opioid receptor gene expression in thalamencephal of IBS model rats,but κ opioid receptor in thalamencephal and both two kinds of opioid receptor in colon rectum had no the significant change.Conclusion EA at acupoint ST-36 could depress chronic visceral hypersensitivity in IBS model rats and these data suggest an active role of opioid system in the effect.
