摘　　要：目的 探讨颅内毛细胞型星形细胞瘤的CT、MRI特征,以提高诊断正确率.方法 回顾性分析21例经手术病理证实的毛细胞型星形细胞瘤的CT、MRI表现.结果 21例中,71.4%发生于20岁以下青少年.小脑蚓部10例,小脑半球4例,鞍区6例,右颞叶1例.分3型：囊肿型2例（9.5%）,囊肿结节型11例（52.4%）,实质型8例（38.1%）.3例瘤内可见钙化,4例可见出血.肿瘤实性部分及壁结节CT呈等密度5例,稍低密度13例,MR T1WI呈等信号8例,稍低信号11例,T2WI呈等信号2例,稍高信号17例;囊性部分CT平扫呈明显低密度,MR T1WI呈明显低信号,T2WI呈明显高信号.增强扫描实性部分及壁结节均明显强化,囊性部分不强化,囊壁明显强化4例,轻度强化2例,无强化9例.肿瘤边界清楚,瘤周无水肿.结论 毛细胞型星形细胞瘤的CT、MRI表现有一定特征性,结合临床资料可在术前做出正确诊断.Objective To study the CT and MRI imaging features of intracranial pilocytic astrocytoma,so that to improve the accuracy of the diagnosis of this tumor. Methods CT and MRI appearances of 21 cases with pilocytic astrocytoma proved by surgery and pathology were retrospectively analyzed. Results Pilocytie astrocytoma usually occurred in adolescent under 20 years old（71.4%）. The lesions located in the vermis of cerebellum in 10, the cerebellar hemisphere in 4, the sellar region in 6 and the right temporal lobe in one. The tumors could be divided into 3 types： cyst type（9.5%）, cyst--node type（52.4%） and predominantly solid mass type（38.1%）. There were calcifications in 3 and hemorrhage in 4 inside the tumors. The solid part of tumors and the wall nodes appeared as isodensity in 5 cases and slightly hypodensity in 13 cases on CT. And the lesions appeared as isointensity in 8 cases and slightly hypointensity in 11 cases on T1WI, isointensity in 2 cases and slightly hyperintensity in 17 cases on T2 WI. The cystic part of tumors appeared as hypodensity on CT and obvious hypointensity signal on T1WI and obvious hyperintensity signal on T2 WI. After injection of contrast medium, the solid part and wall nodes of tumors were enhanced markedly, the cystic part of tumor did not en- hanced, the cystic walls of tumors showed marked enhancement in 4 cases, slight enhancement in 2 cases, did not enhancement in 9 cases. The tumors were sharply demarcated without edema. Conclusion CT and MRI appearances of pilocytic astrocytoma have some characteristics,in combination with clinical data, this tumor can be diagnosed correctly before operation.
