摘 要:研究p53基因点突变在食管癌及癌前病变粘膜细胞癌变过程中的变异作用及p53定量分析与病人预后的关系。方法选取手术新鲜标本及蜡块90例。①DNA提出分析,应用PCR-RFLP和DNA序列分析,对p53基因点突变的方式和类型进行检测、分析;②免疫组化ABC法,根据阳性细胞百分比及着色强度进行积分;利用MIPS-型自动图像分析仪对肿瘤细胞核的形态和DNA含量各参数进行测量;③乳酸脱氢酶染色,利用MPV-2型显微分光光度计检测、分析。结果①p53基因在食管癌,不典型增生组织中点突变频率较高,分别为60%,29.40%,错义突变和移码突变占全部突变的73%。②免疫组化p53阳性表达,在食管癌为60%;不典型增生为42%。p53表达强度与生存期有相关性。③食管癌和不典型增生,与正常粘膜组织比较,乳酸脱氢酶活性增加,P<0.05。结论①p53基因点突变是食管粘膜细胞癌变过程中多因素作用的结果。错义和移码突变可能是其失活的主要原因。②p53定性表达与肿瘤分化程度、大小、预后等无明显关系,而定量表达与病人预后有关,强阳性者,生存期短,预后差。③p53免疫组化结果积分DNA含量改变有相关性。④食管癌组织乳酸脱氢酶活性增高。间质?Objective To investigate the variation action of p53 gene point mutation in esophagus carcinoma,precancerosis and the p53 quantitative analysisrelationship with patients prognaosis.Method Selected 90 cases fresh operative preparations and embeded with was:①DNA advance analysis,used PCR-RFLP and DNA sequential analysis to examine and analyse the fashions and typ53 gene point mutation;②By immunohistochemistry ABC method.Counted accordint to the percentage and degree of stain of positive cells;shaps of tumor nucleus and every parameters of DNA contentwere measured by MIPS type automatic image analysis.③Lactate dehychogenase(LDH)staining,which were analysed by MPV 2 microspectrophometer.Result ①Frequencies of p53 gene point mutation in esophagus carcinoma and dysplasia tissues were higher(60% and 29.4% respectively).Missene mutation and frame shift mutation accuated.for 73 percent of all.②Expressions of p53 positive Immunohistochemistry was 60% in esophagus carcinoma and was 42% in dysplasia.The degree of p53 gene expressions had a relationship with suroioal stage.③Compared esophagus carcinoma and dysplasia with normal mucosa tissue,the activity of LDH increased,P<0.05.Conclusion ①p53 gene point mutation was the effect of multiple factors in the course of canceration of esophagus mucosa cells.Missense mutation and frame shift mutation were possibly the main reason of its mactivation;②p53 qualitative expressions had no apparent relationship with the degree of differentiation,the size the prognosis of tumor,but quantitative expressious were related to prognosis of patcents,the survival stage of strong positive patients was short and prognosis was bad.③The results integral of p53 Immunoshistochemistry had relationship with the change of DNA content.④Activity of LDH of the esophegus carcinoma tissue increased.Stroma monocyte and lymphocyte was the first local defense in preventing the development and diffusion of tumor,the higher amount of LDH means a better prognosis.
