叶绿素显示液在妇科恶性肿瘤淋巴清扫术中的应用

摘　　要：目的探讨叶绿素在妇科恶性肿瘤淋巴清扫术中的应用价值。方法将１４支狗的睾丸或子宫角处注射叶绿素溶液，注射后２，４，６，８，１２，２４，４８，７２，１２０ｈ后分别取其腰淋巴结作光、电镜观察，并按相同时间取股静脉血作叶绿素测定。对２３例宫颈癌患者，术前随机分为宫颈注射叶绿素组１３例和对照组１０例。术中观察记录盆腔淋巴结的染色摘除情况，并取淋巴结作组织切片，显微镜观察。结果叶绿素注射４ｈ后，其在血液中的浓度最高为３６０．６５μｇ／ｍｌ，２４ｈ后，血液测不到叶绿素。狗腰淋巴结平均显色时间为８ｍｉｎ，显色可持需１２０ｈ以上。宫颈癌病人淋巴结染色组，盆腔淋巴结平均摘除３１．３８个，对照组２２．５个（Ｐ＜０．０１）；染色组所用时间为５７ｍｉｎ，对照组为７４ｍｉｎ。结论宫颈注射叶绿素显示盆腔淋巴结。Objective To explore the significance of chlorophyll in lymphadenectomy of female malignant tumors.Method In 14 days,chlorophyll was injected into the testis or theuterine cornu.At the 2th,4th,6th,8th,12th,24th,48th,72th,120th hour,the lumbar lymph nodes were observed by light microscope and electron microscope,the concentration of chlorophyll in the blood was measured.23 patients with cervical carcinoma were divided into coloring group (13 cases) and control group(10 cases) randomly.Chlorophyll solution was injected into cervix in the coloring group.Coloring of pelvic lymph nodes,numbers of the resected nodes and duration of resecting nodes were recorded.Histological sections of resecting nodes were observed by microscope.Result After 4 hours,the concentration of chlorophyll in the blood reached the peak value(360.65μg/ml).After 24 hours,the chlorophyll could not be measured.The average times to stained the lymph nodes was 8 minutes.The average number of the resected lymph nodes was 31.38 in coloring group,22.5 in control group(P<0.01)(P<0.01).Conclusion Injecting chlorophyll into cervix is useful for increasing rate of the resected lymph nodes.
