可乐定治疗阿片类依赖时戒断症状的评定

摘　　要：目的探讨在评定可乐定脱毒的效应时的包括有血压、心率的阿片类戒断症状评定表评定结果的偏倚。方法采用随机双盲对照方法对20例可乐定和20例安慰剂治疗的住院脱毒患者进行观察。结果排除血压和心率指征后，可乐定能明显降低治疗第2、第3天的戒断症状分（P＜0．05），可乐定组第2，3天的减分率分别为42．8％和48．8％，安慰剂组分别为15．9％和28．7％；包括血压和心率指征，可乐定组与安慰剂组间的差异无显著性（P＞0．05）。脱毒后1周内，可乐定能降低坐位收缩压、舒张压及心率。结论建议在可乐定作为阳性对照脱毒治疗药物时，评价其效应的戒断症状表中不计入心血管指征。Objective To examine the deviation of the opiate withdrawal symptom rating scales, includingthe physiological changes such as blood pressure and heart rate, in detoxification study with clonidine. Methods20 patients with clonidine, and 20 patients with placebo were included in a double blind, randomized, controlleddetoxification trial for 11 days. Results Clonidine could significantly reduce the scores of opiate withdrawalsymptom on 2nd and 3rd day without including blood pressure and heart rate signs (P＜0.05). The reductive rateof 2nd or 3rd was 42. 8% and 48. 8% in clonidine group respectively, while placebo group with 15. 9% or28.7% reductive rate. And there was no significan difference with including blood pressure and heart rate signsbetween two groups (P＞0.05). The systclic and diastolic pressures as well as heart rate in the sitting positionwere significantly decreased with clonidine treatment within one-week after detoxification. Conclusions Thecerdiovascular signs are suggested not to be included in the opiate withdrawal symptom rating scale with cloidinetreatment.
