摘　　要：【目的】探讨鼻咽癌病灶18F-FDG SUV在鼻咽癌T分期中的价值,以期能进一步指导临床工作。【方法】收集113例鼻咽癌初诊患者首次放疗前的PET/CT资料,按照T分期分为4组:T1期6例,T2期18例,T3期64例,T4期25例。采用18F-FDG高摄取(SUV≥2.5)为阈值人工勾画感兴趣区(ROI),在XELERIS工作站上自动计算所勾画病灶的大小、SUVmax和SUVmean。分析SUV与T分期的关系。【结果】Kruskal-Wallis秩和检验显示,原发灶大小(Size)、SUVmax、SUVmean在T分期间差异有显著统计学意义(χ2=34.46,17.02,16.62;P均<0.01)。病灶大小和SUVmax、SUVmean之间的相关系数r分别为0.46和0.37,P<0.01。鼻咽癌病灶的大小和SUVmax、SUVmean呈正相关。【结论】18F-FDG PET/CT SUVmax、SUVmean在鼻咽癌T1～T4分期中的差异有统计学意义,并随分期升高逐渐升高,具有一定的临床价值。【Objective】 The aim was to investigate the value of 18F-FDG standardized uptake value （SUV） in T stages of nasopharyngeal carcinoma （NPC）,and to guide clinical work further. 【Methods】 In total 113 histologically diagnosed NPC patients without any radiotherapy underwent 18F-FDG PET/CT in Cancer Center,Sun Yat-sen University between July 2005 and April 2008 were entered into this retrospective study. The patients were grouped according to T stage：T1,6 patients; T2,18 patients; T3,64 patients; and T4,25 patients. Using high 18F-FDG uptake （SUV≥2.5） to draw region of interest （ROI）,the maximum standardized uptake value （SUVmax）,and mean standardized uptake value （SUVmean） of ROI were calculated automatically in XELERIS workstation. The relationship between SUV and T stage was analyzed. 【Results】 Kruskal-Wallis rank sum test showed that the differences of primary tumor size,SUVmax,SUVmean,have a significant statistical significance （χ2 = 34.46,17.02,16.62; P〈 0.01） in the T stages. The correlation coefficients between lesion size and SUVmax,SUVmean were 0.46 and 0.37,respectively （P〈 0.01）. The NPC lesion size and SUVmax,SUVmean were positive correlation. 【Conclusion】 18F-FDG SUVmax,SUVmean have a statistical difference in T1-T4 stages of NPC and gradually increased with the stages ,which have some value in clinic.
