氟尿嘧啶缓释剂联合适形放疗治疗老年食管癌的临床观察

摘　　要：目的观察三维适形放疗联合氟尿嘧啶缓释剂治疗老年食管癌的临床疗效和毒副反应。方法89例老年食管癌患者随机分为两组：观察组45例植入氟尿嘧啶缓释剂后开始放疗；对照组44例行单纯适形放疗。治疗完成后评价疗效和不良反应。结果治疗组近期总有效率93．3％，1、2年生存率分别为84．4％、68．9％；对照组总有效率79．5％，1、2年生存率分别为63．6％、38．6％，两组间差异有统计学意义（P〈0．05）。1、2年局部控制率治疗组为88．9％、80．0％，对照组为70．5％、59．1％，两组间差异有统计学意义（P〈0．05）。治疗过程中出现的不良反应较轻，患者均能耐受，两组间无统计学差异。结论氟尿嘧啶联合三维适形放疗治疗老年食管癌安全、有效，值得临床进一步研究。Objective To observe the clinical effect and toxicity of three dimension conformal radiotherapy （3D-CRT） combined with slow-released fiuorouracil in elderly esophageal cancer. Methods Eightynine elderly esophageal cancer patients were divided two groups：35cases of the treatment group were given 3D- CRT after slow-released fluorouracil was implanted ,44 cases of control group received 3D-CRT alone. The clinical effect and toxicity was assessed after treatment. Results The recent total effective rate of the treatment group was 93.3 %, thel and 2 year survival rate of the treatment group was 84.4% and 68.9% respectively; The recent total effective rate of the control group was 79. 5% ,the land 2-year survival rate of the control group was 53.6% and 38.6% respectively. There was statistical significant difference between the two groups （ P 〉 0. 05 ）. The local control rate of 1 and 2-year in treatment groups was 88. 9% and 80. 0% respectively; the local control rate of 1 and 2-year in control groups was 70. 5% and 59.1% respectively; There was statistical significance difference between the two groups. During the treatment, the patients had light toxical effect and could impatient. Conclusion Three dimension conformal radiotherapy （3DCRT） combined with slow-released fluorouracil in elderly esophageal cancer is safty,effective and valuable clincial study.
