赔偿性神经症患者MMPI及相关心理量表特征分析

摘　　要：目的探讨赔偿性神经症在MMPI及相关心理量表上的表现特征及其与普通神经症、诈病的差异。方法对涉及赔偿纠纷后诊断为赔偿性神经症、诈病及普通神经症用明尼苏达多相人格测验、二项必选数字记忆测验、简易精神症状自陈量表进行检测。结果 (1)三组研究对象在MMPI的多个分量表分值差异均有显著性,包括F(稀有回答)、Hs(疑病)、D(抑郁)、Hy(癔病)、Pd(精神病态)、Pa(偏执)、Pt(精神衰弱)、Sc(分裂)等(P(0.01或(0.05),赔偿性神经症的得分值介于诈病和普通神经症之间。(2)三组的简易精神症状自陈量表总分差异显著(P(0.01);两两比较结果显示各组间差异均有显著性(P(0.05)。(3)三组研究对象用二项必选数字记忆测验的判别有主观故意夸大智力、记忆缺损的人数比例差异显著(P(0.01),诈病组故意答错的比例较大,普通神经症组多数为合作,而赔偿性神经症组合作、随机作答和故意答错的比例差别不大。结论赔偿性神经症在多种心理量表上,既具有神经症的某些表现,又具有类似诈病的心理特征。Objective To explore characters and differences of MMPI and related assessment scales among compensation neurosis, typical neuroses and malingering. Methods The subjects including the cases with compensation neurosis or malingering involved forensic problems and the patients with typical neuroses in general psychiatric clinic were tested by MMPI, Forced - Choice Digit Memory Test and Malingering Test. Results （1）There were significant differences among three groups in the most dimensions ofF, Hs, D, Hy, Pd, Pa, Pt, Sc in MMPI （P 〈 0. 01 or 〈 0.05）. The scores of MMPI in the subjects of compensation neurosis were between that of typical neuroses and malingering. （2） There were significant differences among three groups in the total score of Malingering Test（P 〈 0.01 ）. （3） There were significant differences among three groups in the cooperation degree of Forced -Choice Digit Memory Test（P 〈 0.01 ）. Conclusion The compensation neurosis is the psychological state which is between malingering and typical neuroses.
