摘 要:目的研究应用化疗药后肝在声像图上回声增强的病理学基础。方法 40只新西兰白兔,随机分为2组,每组20只。一组给予旧化疗药(旧药组),一组给予新化疗药(新药组)。化疗方案选用乳腺癌治疗方案,新旧药均应用3个周期。每只兔用药前后均查肝功能、血脂并作肝脏超声检查。超声图像用photoshop6.0软件中的直方图功能分析回声强度,最后做兔肝脏病理切片观察。结果新、旧药组兔肝用药前后的超声回声强度比较,差异均有统计学意义(P<0.05);2组用药后兔肝的回声强度比较差异也有统计学意义(P<0.05)。病理改变包括兔肝细胞变性,炎细胞浸润,局灶钙化,纤维母细胞增生,甚至局灶性坏死等。用药后新旧、药组的兔肝功能均有异常增高。结论应用化疗药可引起肝脏声图像异常,是肝细胞变性、坏死及钙化等病理改变所造成。新化疗药较旧化疗药对兔肝的损害轻。Objective To investigate the pathological basis of the increase of hepatic echo levels in livers after administration with chemotherapeutants,and the effects of old and new chemotherapeutic agents on rabbit liver.Methods 40 rabbits were randomly divided into two groups.One group was given old chemotherapeutants injection(cyclophosphamide plus adriamycin),the other group was given new chemotherapeutants injection(taxol plus epirubicin)for 3 cycles respectively.Before and after administration,the hepatic function,blood lipids and liver ultrasound were performed.The adjuvant therapy for breast cancer was chosen.The sonograms were stored in an ultrasound workstation for offline analysis.All the data were statistically analyzed(t test)with SPSS 13.0 software.Finally,all the rabbits were executed,the livers were taken out and stored in formalin solution for pathological observation.The results of pathological changes were analyzed with Fisher's exact test.Results The average rabbit hepatic echo levels before and after administration in old drug group were 25.64±7.82 and 46.63±7.54 respectively,and there was a significant difference before and after the injection(P〈0.05),however,in new drug group,which were 23.02±5.68 and 30.48±6.44 respectively,and there was also a significant difference before and after the injection(P〈0.05).But the echo levels of rabbit liver in old drug group after injection were significantly higher than those in new drug group(P〈0.05).The pathological changes in rabbit liver were mainly cloudy swelling and hydropic swelling in both groups,in addition,the lesions included inflammatory cells infiltration,focal necrosis,ballooning degeneration,fatty degeneration,fibroblast hyperplasia,focal calcification,etc.There were significant differences in irreversible pathological changes between the two groups(P〈0.05).All the rabbits in both groups showed an increase of AST or ALT values after administration.Conclusion After injection with chemotherapeutants,the rabbi
