胸腔灌注顺铂联合静脉化疗和三维适形放疗治疗肺癌胸腔积液

摘　　要：目的观察胸腔灌注顺铂联合静脉化疗和三维适形放疗(3DCRT)治疗肺癌胸腔积液的疗效。方法肺癌胸腔积液24例均给予静脉化疗和顺铂胸腔灌注;同时进行肺部及纵隔肿块的3DCRT。结果 24例肺癌胸腔积液中,CR 10例,PR 12例,总有效率91.7%。主要毒副反应为骨髓抑制、胃肠道反应和急性放射性肺损伤。结论胸腔灌注顺铂联合静脉化疗和3DCRT治疗肺癌胸腔积液疗效较好,毒副反应可耐受。Objective To evaluate the efficacy of thoracic cavity perfusion with cisplatin combined with chemotherapy and three dimensional conformal radiation therapy（3DCRT） for pleural effusion of patients with lung cancer.Methods twenty four lung cancer patients with pleural effusion were treated with chemotherapy,thoracic cavity perfusion with cisplatin,and 3DCRT targeting at lung and mediastinum lesions.Results Of the 24 lung cancer patients with pleural effusion,CR was observed in the 10 patients,and PR in the 12 patients;the total response rate was 91.7%.The main toxicities were marrow suppression,gastrointestinal reaction and acute radiation pneumonitis.Conclusion Thoracic cavity perfusion with cisplatin combined with chemotherapy and 3DCRT has good effects on lung cancer patients with pleural effusion,and its toxicities are tolerable.
