逍遥散联合帕罗西汀对老年抑郁症疗效及HPA轴功能的影响

摘　　要：目的:观察逍遥散联合帕罗西汀治疗老年抑郁症的疗效并探讨其神经内分泌学机制。方法:将60例老年抑郁症患者随机分为研究组(逍遥散合并帕罗西汀治疗组)和对照组(帕罗西汀治疗组)。研究组采用逍遥散合并帕罗西汀治疗6周,对照组单纯使用帕罗西汀治疗6周,在治疗前后分别作地塞米松抑制试验(DST),并检测血浆皮质醇和促肾上腺皮质激素(ACTH)的浓度。结果:治疗后两组临床疗效和ACTH、CORT值均有明显改善,研究组尤甚(P<0.05)。结论:逍遥散联合帕罗西汀治疗老年抑郁症和单独使用帕罗西汀治疗相比,能够更好地改善患者的抑郁状态,其机理可能与抑制下丘脑一垂体一肾上腺(HPA)轴功能亢进有关。Objective：To observe the clinical effectiveness of Xiaoyao Powder combined with paroxetine and explore the neuroendocrine mechanism in senile depression patients.Methods：60 cases of senile depression were randomly divided into treatment group treated with Xiaoyao Powder combined with paroxetine for 6 weeks and control group treated with paroxetine for 6 weeks.Dexamethasone suppression test（DST） before and after treatment were made and then the plasma cortisone and corticotrophin（ACTH） were determined.Results： After 6 weeks,therapeutic effect and the plasma concerntrations of ACTH、CORT were significantly improved in both groups in comparison to that of before treatment,and treatment group was better than control group（P0.05）.Conclusions： Xiaoyao Powder combined with paroxetine can improve therapeutic effects and the probable mechanism is that Xiaoyao Powder combined with paroxetine can regulate the balance of HPA more effectively.
