溃疡性结肠炎患者血小板膜糖蛋白Ⅰb/Ⅸ/Ⅴ复合物与疾病严重程度、血小板激活的相关性

摘　　要：目的探索溃疡性结肠炎(ulcerative colitis,UC)患者血小板膜糖蛋白Ⅰb/Ⅸ/Ⅴ(glycoproteinⅠb/Ⅸ/Ⅴ,GPⅠb/Ⅸ/Ⅴ)复合物与疾病严重程度、血小板激活的关系。方法选取了54例活动期UC患者和35名健康志愿者,采用美国胃肠病学会制订的临床指南对活动期UC患者的疾病严重程度进行分级。用比浊法检测UC患者和志愿者血小板最大聚集率(maximum agglutination rate,MAR)。采用流式细胞仪检测血小板膜糖蛋白Ⅰb/Ⅸ/Ⅴ复合物、血小板激活标志物血小板α颗粒膜糖蛋白(CD62P)和血小板表面纤维蛋白原受体(PAC-Ⅰ)的水平。结果活动期UC患者血小板MAR、CD62P和PAC-Ⅰ的阳性表达率较正常对照组明显升高,而GPⅠb/Ⅸ/Ⅴ复合物的表达显著降低。在轻、中以及重度活动期UC患者中,血小板MAR、GPⅠb/Ⅸ/Ⅴ复合物的表达、CD62P和PAC-Ⅰ的阳性表达率差异具有统计学意义。相关性分析显示血小板GPⅠb/Ⅸ/Ⅴ复合物的表达与血小板激活指标MAR、CD62P、PAC-Ⅰ呈负相关。结论血小板GPⅠb/Ⅸ/Ⅴ复合物与血小板激活相关,可能在UC的发展中起作用并反映UC严重程度。Objective To investigate the correlation of platelet membrane glycoprotein Ⅰb/Ⅸ/Ⅴ complex with clinical severity and platelet activation in patients with ulcerative colitis（UC）. Methods The enrolled in this study included 44 UC patients and 35 health volunteers. The disease severity was assessed clinically according to the practice guidelines of the American College of Gastroenterology. The maximum agglutination rate （MAR） was assayed by platelet aggregometer. GP Ⅰb/Ⅸ/Ⅴ complex,CD62P and PAC Ⅰ were detected by using flow cytometry. Results The expression of GP Ⅰb/Ⅸ/Ⅴ complex in platelets in UC group was decreased markedly as compared with control group. MAR and the positive expression rate of CD62P and PAC- Ⅰ in active UC patients were significantly higher than in control group. Statistical analysis showed there was significant difference in MAR,the expression of GP Ⅰb/Ⅸ/Ⅴ complex,the positive expression rate of CD62P and PAC Ⅰ among different groups of UC patients. Correlation analysis revealed that MAR, the expression of GP Ⅰb/Ⅸ/Ⅴ complex, and the posi tive expression rate of CD62P and PAC -Ⅰ were positively correlated with the severity of the disease. Conclusion These indicators can be taken as makers of disease severity. GP Ⅰb/Ⅸ/Ⅴ complex may play a certain role in the development of UC.
