精神分裂症既往睡眠状态与脑诱发电位研究

摘　　要：目的探讨精神分裂症患者既往睡眠状态与临床症状、脑诱发电位的关系。方法收集203例精神分裂症患者和192名健康者既往睡眠时间与脑诱发电位资料,并用阳性和阴性症状量表(PANSS)评定患者的临床症状。结果精神分裂症组与健康对照组的既往睡眠时间差异有统计学意义。精神分裂症患者当中,与睡眠>8h/d相比,睡眠<5h/d组的PANSS总分、一般症状分、注意障碍分、意志障碍分、主动回避社交分显著升高;与睡眠5～8h/d相比,睡眠<5h/d组AEP-P2b潜伏期显著缩短。精神分裂症患者既往睡眠时间与PANSS总分、一般症状分、抽象思维障碍分呈负相关;精神分裂症患者既往睡眠时间与脑诱发电位指标间未发现相关性。结论既往睡眠不足可能与精神分裂症患者注意障碍、意志障碍、主动回避社会有关,可能是精神分裂症脑功能损害的因素之一。Objective To explore the correlation among past sleep hours, clinical symptoms and evoked brain potentials in patients with schizophrenia. Methods Evoked brain potentials and past sleep hours were measured in 203 schizophrenic patients and 192 normal controls. The clinical symptoms were measured with the Positive and Nega- tive Syndrome Scale （PANSS）. Results There was a significant difference in past sleep hours between schizophrenic patients and normal controls. Among schizophrenic patients, those who slept 5 hours or less per day had remarkably higher scores in PANSS, general symptoms, disordered concentration, dysboulia and social withdrawal than those who slept 8 hours or more per day. Compared with those who slept 5 to 8 hours per day, patients who slept less than 5 hours per day had significantly shorter AEP-P2b latency. Past sleep hours in schizophrenic patients were negatively correlated with scores of general symptoms, disturbance of abstract thinking and total PANSS score. Conclusion Insufficient sleep in the past may have relationship with general symptoms, disturbance of abstract thinking, dysboulia, social withdrawal and disturbance of attention, and therefore may be a factor leading to brain functional lesion in schizophrenic patients.
