门静脉双氧水造影在门脉化疗中的意义

摘  要:目的:利用门静脉穿刺双氧水造影观察原发性肝癌患者肿瘤门脉血供情况及效果。方法:14例原发性肝癌患者在超声导向下,穿刺进入门静脉,迅速注入2%双氧水2~4ml,在超声波监视屏下观察门静脉内双氧水分解后,气泡反射光点的多少及走行方向。结果:14例病人中,有10例肿块光产、增强,轮廓显示清晰。2例原在超声波下边界不清的肿块屯清楚的显示了其轮廓。1例癌肿气泡显示不清.另一例肝内肿瘤天气泡聚积。造影后均无不良反应。结论:双氧水造形可显示门脉血流的走行方向及肝癌肿块的门脉血供情况并使肿块的轮廓显示增强。同时提示不同病人肝癌肿块的门脉血供是有差异的,故不加区别的对所有原发性肝癌行门脉化疗是否合适值得商酌。Conditions and results of blood supply in portal vein to tumor of the patients with primary liver cancer have been observed by portal radiography with hydrogen peroxide.Method:With ultrasonic director, 14 cases were given portal puncture,then 2- 4ml hydrogen peroxide was instantly injected into the vein in order to observe the number and direction of the reflecting ligting points of the areated bubbles produced from the decomposed hydrogen peroxide, with a ultrasonic monitor. Results: Among the 14 cases, 10 cases were shown the strengthened lighting points and clear outline of the tumor. 2 cases which were with vaguddemonstration of the border of the tumor under only ultrasonic observation were also shown clear outline. 1 case was shown no clear bubble,and the last was shown no bubble accumula-tion inside the tumor, No side affects occured to all the cases after radiography. Conclusion:This method can indicate the direction of blood current in portal vein,the conditions of blood supply in portal vein to the tumor,and reinforced display of the outline of the tumor. They also suggest the individual differences in portal blood supply to tumors. It is still discussible if it is suitable that portal chemotherapy should be given to every patient with primary liver cancer.
