摘  要:目的探讨糖尿病肾病患者血清中基质细胞趋化因子1α(SDF-1α)的水平,与血糖、血脂、胰岛素抵抗、高敏C反应蛋白(hsC-RP)等的相关性。方法选取健康对照(NC)组31名,2型糖尿病(T2DM)组151例。检测hsC-RP、HbA_1c、TG、TC等生化指标,ELISA法检测血清SDF-1α,计算HO-MA-IR。结果 T2DM组SDF-1α明显高于NC组(P<0.01)。伴有肾病组高于不伴有肾病组,大量白蛋白尿组高于微量白蛋白尿组,但差异无统计学意义(P>0.05)。SDF-1α与hsC-RP、HbA_1c、HOMA-IR和尿白蛋白/肌酐(UAlb/Cr)呈正相关,与血脂不相关。T2DM组hsC-RP与HbA_1c、HOMA-IR、UAlb/Cr呈正相关(P<0.01)。结论 T2DM患者SDF-1α水平升高,可能参与糖尿病肾病的炎症和免疫反应过程。Objective To explore the role of SDF-1α in pathogenesis of type 2 diabetic nephropathy and its relation with BG, blood lipid,insulin resistance,and hsC-RP. Methods Serum SDF-1α in 31 normal subjects and 151 subjects with type 2 diabetic nephropathy was detected by ELISA method. Results (1)The serum SDF-1α was higher in patients with type 2 diabetic nephropathy than in healthy controls( 3. 01±0. 68 and 1.07± 0.35ng/ml, P 〈 0.01 ). The group with macroalbuminurine was higher than the group without microalbuminuria, but the difference was without statistical significance. (2) Serum SDF-1α was correlated positively with high-sensity C reactive protein (hsC-RP) ( r= 0. 432, P% 0.05), HbA1 c ( r= 0. 213, P〈0. 05 ), HOMA-IR(r=0. 265, P%0. 05), UA1b/cr(r= 0. 29, P〈0. 05) and had no correlation with blood lipids. (3) Serum hsC-RP was correlated positively with HbA1c (r=0. 91,P〈0. 01), HOMA-IR(r= 0. 93,P〈0. 01), UAlb/cr(r=0.67, P〈0.01 ). Conclusions SDF-1α plays an important role in the pathogenesis of type 2 diabetic nephropathy.
