慢性闭角型青光眼激光虹膜周切术后前房角改变的超声测量

摘　　要：目的：动态地了解慢性闭角型青光眼（慢闭青）行ＹＡＧ激光虹膜周切术（ＹＬＩ）前、后的前房角定量改变，和虹膜膨隆的形态改变。方法：随机对确诊为慢闭青的门诊患者１１人１８眼，经房角镜检查按Ｓｃｈｅｉｅ分类法均为窄２～窄３、４之间，裂隙灯检查前房下角均＜１／３ＣＴ。眼压＞２１ｍｍＨｇ，Ｃ／Ｄ０．２～０．５，接受药物治疗的患者。在行ＹＬＩ之前和ＹＬＩ之后，用ＡＣＵＳＯＮ１２８ＸＰ／１０型电脑声像仪各行一次超声房角测量，同时观察虹膜膨隆形态的改变。结果：超声测量显示手术前、后，前房角的平均宽度有明显改变，ＹＬＩ前为１５．２２°±５．８７°，ＹＬＩ后为２４．４４°±５．９６°。ＹＬＩ后周边虹膜不再向前膨隆，前房下角＞１／３ＣＴ。结论：ＡＣＵＳＯＮ１２ＸＰ／１０型电脑声像仪可用以定量测量活体前房角宽度，超声测量显示慢闭青施行激光虹膜周切术后，前房角明显增宽，虹膜膨隆变平。Objective:A quantitative dynamic study of the anterior chamber angle and observation of the profile of iris bombe of the primary chronic angle closure glaucoma(CACG)before and after the YAG laser iridectomy(YLI).Methods:A randomized selected 11 cases 18 eyes of GACG patients in out patient department.They were all with narrow angle of N2～N3 or N4(Scheie),the lower anterior chamber angle examined by slitlamp biomicroscope were<1/3CT.The IOP all were>21 mmHg and with the C/D ratio 0 2～0 5.All patients were accepted antiglaucoma drug treatment.The Acuson 128XP/10 computed sonography was used to measure the width of anterior chamber angle prior and after the performance of YLI procedure and to observe the changing of the profile of iris bombe.Results:The degrees of the mean anterior chamber angle width in our study pre and post of YLI respectively were 15 22°±5 87° and 24 44°±5 96°.After YLI the peripheral iris shown no more bowing anteriorly and the angle in slitlamp examine were>1/3 CT.Conclusion:The Acuson 128XP/10 Computed Sonography can be used for quantitatively measuring the width of anterior chamber angle.It was showing that in CACG eyes after YLI the anterior chamber angles became widened than before,and at the same time that the configuration of iris bombe became flattened
