摘 要:目的 建立一种输尿管战创伤动物模型.方法 使用自行研制的火器弹片伤制造模具,分别对模型建立组9条比格犬的一侧输尿管制造火器弹片伤,并于30 min后直接清创缝合.分别于术后40、80、120 d进行静脉肾盂造影、肾图检查;并分批处死比格犬,取出输尿管行组织形态观察,初步评估该模型的可行性和有效性.结果 模型建立组9条比格犬,术后损伤侧出现肾盂、输尿管积水并逐渐加重;肾脏分浓缩率比值(未损伤侧/损伤侧)逐渐增高,半排时间比值(未损伤侧/损伤侧)逐渐下降,差异有统计学意义;损伤侧输尿管吻合处瘢痕增生、狭窄.结论 该输尿管火器弹片伤比格犬模型,损伤侧术后出现肾盂、输尿管积水并逐渐加重;损伤侧较之未损伤侧,肾小管的浓聚功能下降,肾脏的半排时间延长;损伤侧输尿管吻合处狭窄、瘢痕纤维排列紊乱.证明它是一种可行、有效的输尿管战创伤动物模型.Objective A new canine model of firearm fragment wounds on ureters was built up and tested for its viability and efficacy. Methods A hand-made device was used to make firearm fragment wounds unilaterally on ureters of 9 Beagle dogs in model build-up group. The wounds were debrided and sutured 30 min later. On the postoperative day 40, 80 and 120, intravenous pyelography ( IVP) and radioactive renography were done. Then Beagle dogs were all sacrificed, after which ureters were harvested for histological examination. The viability and efficacy of the aforesaid canine model was evaluated. Results In model build-up group, the hydronephrosis and hydroureter occurred and got worse postoperatively on the wounded side in all 9 Beagle dogs. The renal patial concertration index ratio of bilateral kidneys (unwounded side/wounded side) was increased. The half time of kidney washout ratio (unwounded side/wounded side) was decreased. Hyperplastic scar and stenosis were observed in the anastomoses where ureteroureterotomy had been performed. Conclusion After the firearm fragment injury was made on the ureter of Beagle dogs, hydronephrosis and hydroureter occurred at the wounded side and got worse as time went on. Compared with the situation on the unwounded side, on the wounded side the renal uptake became slower and the half time of kidney washout was extended. There were stenosis and irregular fibers in the anastomoses where ureteroureterotomy had been performed. This proved the canine model of firearm fragment wounds to be viable and effective.
