摘  要:目的:研究前列腺特异性膜抗原(PSMA)、雄激素受体(AR)在膀胱移行细胞癌中的表达及其诊治中的临床意义。方法:应用免疫组织化学SP法分别检测75例膀胱移行细胞癌组织及15例正常膀胱黏膜组织中PSMA、AR的表达情况。结果:15例正常膀胱黏膜组织中均未见PSMA和AR的表达,而在75例膀胱移行细胞癌组织的阳性表达率分别为28.0%和21.3%,与正常膀胱黏膜组织相比差异均具有统计学意义(均P<0.05);不同临床分期和病理分级患者PSMA阳性表达差异无统计学意义,而其在复发性膀胱癌患者的表达高于初发性膀胱癌患者,差异具有统计学意义(P<0.01);不同临床分期患者AR的阳性表达差异具有统计学意义(P<0.05)。在膀胱移行细胞癌中,PSMA与AR的表达相关联(r=0.328,P<0.01)。结论:PSMA和AR能客观反映膀胱移行细胞癌的生物学行为,分别对判断恶性程度、预后具有指导意义,且两者可能在膀胱移行细胞癌的发生、发展过程中具有协同作用。Objective: To investigate the expression and clinical significance of prostate specific membrane antigen (PSMA) and androgen receptor (AR) in bladder transitional cell carcinoma (BTCC). Methods: Immunohistochemistry (SP method) was used to detect the expressions of PSMA and AR in BTCC (n = 75) and normal bladder mucosa(n = 15). Results: No expressions of PSMA and AR were detected in normal bladder mucosa, and the positive expression rates of PSMA and AR were 28.0% and 21.3% in 75 cases of BTCC respectively. There were significant differences in positive expression rates of PSMA and AR between BTCC and normal bladder mucosa (P 0.05). There were no significant differences in positive expression of PSMA between the different clinical stages and pathological grades. There was significantly higher positive expression of PSMA in patients with recurrent bladder cancer than that of patients with early-onset bladder cancer (P 0.01). There was significant difference in positive expression rate of AR in BTCC with the different clinical stages (P 0.05). There was a positive correlation between the expression rates of PSMA and AR (r = 0.328, P 0.01). Conclusion: The expression of PSMA and AR may be regarded as important markers in biological characters and prognosis of BTCC, which may interact with one another in the development of BTCC.
