摘  要:目的探讨容积CT(VCT)三期增强扫描结合内镜逆行胰胆管造影(ERCP)图像后处理技术诊断肝门胆管癌的价值。方法对20例经手术病理证实的肝门胆管癌患者的VCT三期增强扫描及ERCP图像进行分析,利用工作站对原始数据进行图像后处理,包括多平面重组(MPR)、容积再现(VR)、CT仿真内镜(CTVE)等,判断肿瘤的部位和侵及范围,肝动脉和门静脉受侵情况,是否有局部淋巴结及脏器转移,并将Bismuth-Corlette分型及术前评估结果与手术结果进行对照分析。结果 20例患者均接受VCT三期增强及ERCP扫描,并进行图像后处理,定性诊断准确率为100%(20/20),定位分型准确率为80.00%(16/20)。应用VCT增强扫描结合ERCP的图像后处理技术诊断肝门胆管癌优于单纯VCT增强扫描。结论 VCT增强结合ERCP后图像后处理技术可作为肝门胆管癌诊断及术前评估的首选方法。Objective To explore the value of enhanced VCT and post-processing techniques of ERCP in the diagnosis of hilar cholangiocarcinoma.Methods Twenty patients with hilar cholangiocarcinoma confirmed histopathologically underwent three-phase enhanced VCT and ERCP scan.The images were analyzed retrospectively.The primary data of all patients underwent image post-processing(MPR,VR,CTVE) with the workstation.The location,extension of tumor,infiltrated hepatic artery(HA) and portal vein(PV),lymph node and liver metastatic were observed.The result of Bismuth-Corlette classification and preoperative evaluation was evaluated with operation.Results The accuracy rate of VCT qualitative diagnosis was 100%(20/20),and the accuracy rates of identifying the classification location was 80.00%(16/20).Enhanced VCT and post-processing techniques of ERCP were more accurate than simply enhanced VCT in the diagnosis of hilar cholangiocarcinoma.Conclusion Biliary tree and vessel reconstruction with three-phase enhanced VCT and post-processing techniques of ERCP could be the top choice in the diagnosis and preoperative evaluation of hilar cholangiocarcinoma.
