摘 要:目的:探讨宫颈癌患者三维适形放疗或同步放化疗对性激素的影响。方法:66例宫颈鳞状细胞癌的初发初治患者均行三维适形放疗(3DCRT)外照射和后装治疗。45例行1周期PF方案化疗,顺铂(DDP)40mgd1-3,5-氟尿嘧啶(5-FU)500mgd1-5。66例患者中8例绝经。在治疗前及治疗结束后分别用放射免疫法检测性激素。结果:所有66例患者经放疗或放疗加化疗后,FSH、LH升高到近3倍(P<0.05),E2下降近1半(P<0.05),P下降到近1/3(P<0.05)。45例患者经放疗加化疗治疗后,除了FSH、LH升高到3倍(P<0.05),E2下降近1半(P<0.05),P下降到近1/3外(P<0.05)外,T也下降(P<0.05)。21例未加化疗的患者治疗后,FSH升高到2.4倍(P<0.05),LH升高到近3倍(P<0.05),E2下降近1半(P<0.05)。8例绝经患者治疗后,FSH、LH、E2、P及T的变化不显著。结论:同步放化疗较单独放疗对宫颈癌患者卵巢分泌功能的影响大。Objective:To explore the effect of three-dimensional conformal radiotherapy or radiotherapy and concurrent chemotherapy on sex hormones of the patients with cervical cancer. Methods: Sixty six primary patients of cervical cancer were enrolled in the trial. All patients were treated by 3DCRT(three-dimensional conformal radiotherapy) external beam radiation and intracavitary brachytherapy after-loading therapy. Forty five patients were treated by chemotherapy, one period of cisplatin(CDDP) and 5-fluorouracil(5-FU) regimen, CDDP 40mg d1-3, 5-FU 500mg d1-5. Eight patients were menopause. Sex hormones were measured by radioimmunity before and after treatment . Results: FSH and LH of sixty six patients after treatment increased nearly three times than that of before treatment(P〈0.05), E2 decrease nearly a half(P〈0.05), P decrease nearly two-third(P〈0.05). Except FSH and LH of forty five patients after radiotherapy and chemotherapy increase nearly three times than that of before treatment(P〈0.05), E2 decrease nearly a half(P〈0.05), P decreased nearly two-third(P〈0.05), T decreased also(P〈0.05). In twenty one patients without chemotherapy, FSH increased 2.4 times(P〈0.05), LH increased nearly three times(P〈0.05) and E2 decreased nearly a half than that of before treatment(P〈0.05). FSH, LH, E2, P and T had no significant changes in eight menopausal patients. Conclusion: Radiotherapy and concurrent chemotherapy have significant effects on sex hormones of the patients with cervical cancer than that of radiotherapy alone.
