应用锻炼指导光盘提高食管癌患者呼吸功能的效果观察

摘　　要：目的探讨呼吸功能锻炼指导光盘对食管癌手术患者呼吸功能及胸部并发症发生的影响。方法将76例拟开胸行食管癌根治手术的患者随机分为对照组和观察组各38例,对照组予常规口授方法指导呼吸功能锻炼及正确排痰方法,观察组除了常规方法指导锻炼外,在护士指导下跟随自制呼吸功能锻炼指导光盘播放内容同步练习呼吸训练和有效排痰方法。于术前1 d评价两组患者呼吸功能锻炼的配合程度、准确性,入院后第2天和术前1 d比较两组患者锻炼前后的动脉血气值、肺功能,以及两组患者术后胸部并发症的发生情况。结果观察组患者呼吸功能锻炼配合程度和准确性均较对照组高;术后胸部并发症少于对照组,动脉血气分析值和肺功能两组患者入院第2天比较差异均无统计学意义,但术前1 d除PaCO2值两组比较差异无统计学意义外,观察组患者PaO2、SpO2及肺功能均高于对照组,差异有统计学意义(P<0.01)。结论应用呼吸功能锻炼指导光盘指导食管癌患者术前呼吸功能锻炼能有效改善患者肺功能,促进术后康复,减少术后并发症的发生。Objective To explore the effect of the CD-ROM of Systematic Guidance on Respiratory Function Exercise on patients with esophageal cancer.Methods 76 patients with radical correction through chest incision for esophageal cancer were randomly divided into control group and experimental group,38 in each group.Patients in control group received conventional methods to guide respiratory training and those in experimental group were guided by playing the intuitive VCD discs.The degree of patients ’ cooperation and accuracy in respiratory function exercise,lung function,arterial blood gases and postoperative complications were all evaluated.Results There were significant differences on the lung function and arterial blood gases exist between two groups（P 0.01）.Further more,the degrees of cooperation and accuracy in experimental group were higher than those in control group.Conclusion Applying the CD-ROM of Systematic Guidance on Respiratory Function Exercise to guide respiratory training can improve the lung function in patients with esophageal cancer,promote the rehabilitation of postoperative lung function and reduce the postoperative complications.
