加兰他敏对精神分裂症认知功能障碍的影响

摘　　要：目的探讨加兰他敏治疗精神分裂症认知功能障碍的疗效及安全性。方法将慢性精神分裂症患者随机分为两组，一组予加兰他敏治疗，另一组为对照组，两组均用抗精神病药物利培酮治疗，疗程16周。治疗前和治疗第16周末进行认知功能、PANSS和TESS测定。结果（1）治疗前，除WCST的随机错误数外，患者组各项认知功能指标均显著差于正常对照（P〈0．01）；（2）治疗后，患者组PANSS总分显著下降（P〈0．01）；认知功能除WCST的随机错误数外较治疗前有所改善（P〈0．05），但总体仍显著差于正常对照（P〈0．01）。加兰他敏组发生不良反应者占受试者的23％，多数为轻中度、一过性，停药后或对症处理后可很快缓解。结论加兰他敏安全有效，对改善精神分裂症患者的认知功能有显著疗效。Objective To evaluate the efficacy and safety in treatment of patients with chronic schizophrenia Patients with chronic schiz- ophrenia. Methods 120 schizophrenic patients were collected. Cogn itive functions of all patients were evaluated with 8 neuropsychological tests which included in semantic fluency, visual reproduction, association learning, digit span, similarity, comprehension, block design , Wisconsin card sorting test（WCST） before and after 16 -week atypical antipsychotics treatment. Clinical symptoms were assessed with Positive and Negative Syn- drome Scale （PANSS）. Safety issues, including vital sign s. 1 ab assays and ECG examinations were measured. Results （ 1 ） Before treatment, the performance of cognitive tests in schizophrenic group was sign ificantly poorer than that in norm al group except for random errors of WCST（ P 〈 0. 01 ） ; （2）After treatment, total scores of PANSS decreased significantly （P 〈 O. Ol ） ;the perform ance of cognitive tests in schizophrenic group was improved except for random errors of WCST（ P 〈 0. 05 ）, but still significantly poorer than that in normal group in general （ P 〈 0. 01 ）. Galantamine had little influence on vital signs and lab assays. Conclusion Safe and well tolerated,galantamine improves the cognition and psychi- atric symptoms of patients with chronic schizophrenia
