小切口白内障囊外摘出联合巩膜隧道内小梁切除术

摘　　要：目的探讨小切口白内障囊外摘出联合巩膜隧道内小梁组织切除治疗青光眼合并白内障患者的临床疗效。方法对33例(33眼)青光眼合并白内障施行小切口白内障囊外摘出及人工晶状体植入,再借助小梁剪在巩膜隧道内直视下完成小梁和周边虹膜的切除。随访3～6个月,并与对照组单纯青光眼32例(32眼)的传统小梁切除术相比较。结果术后视力、眼压及并发症,与对照组的差异无统计学意义(t=1.76,P=0.067)。结论小切口白内障囊外摘出联合巩膜隧道内小梁切除术疗效好,而且操作简便、安全。Objective To survey the clinical efficacy of the surgery of small incision extracapsular cataract extraction combined with trabeculectomy in the meshwork resection scleral tunnel for the treatment of glaucoma and cataract patients. Methods 33 cases （33eyes） of glaucoma and cataract patients were treated by the small incision cataract extraction and intraocular lens implantation; and then,in the scleral tunnel,the resection of trabecular meshwork and peripheral iris was finished under direct vision with trabecular shears. The changes of the eyesight,intraocular pressure and complications after the surgery were followed up for 3 ～ 6 months,and clinical com m＇ision with traditional trabecular resection of 32 cases（32 eyes） with glaucoma. Results Of the two groups,eyesight,intraocular pressure and complications after the surgery had no significant difference （t = 1. 76,P = 0. 067） . Conclusion The surgery of small incision cataract extraction jointing scleral tunnel trabeculectomy has a good efficacy with easy and safe operation. Besides,it can decrease the expenses of the surgery. This method deserves further clinical applications.
