鼻咽癌患者血浆与PBMC EBV-DNA定量测定的临床意义

摘　　要：目的探讨血浆和外周血单个核细胞(PBMC)EBV-DNA定量测定对鼻咽癌的诊断价值。方法采用荧光定量PCR法,对56例鼻咽癌患者、60例其它种类肿瘤患者及60例健康对照者血浆与PBMC EBV-DNA进行定量测定,比较鼻咽癌患者与其它种类肿瘤患者及健康对照者血浆和PBMC中EB病毒阳性检出率及EBV-DNA拷贝含量水平的差异,以及鼻咽癌不同分期EBV-DNA含量之间的差异。结果鼻咽癌患者血浆EBV-DNA阳性检出率(73.2%)明显高于其它肿瘤患者组(23.3%)及健康对照组(11.7%),结果之间存在显著性差(P<0.01);鼻咽癌患者PBMC EBV-DNA的阳性率(89.3%)明显高于其它肿瘤患者组(8.3%)及健康对照组(5.0%),其阳性率之间存在显著性差异(P<0.01);鼻咽癌组的血浆及PBMC EBV-DNA浓度明显高于其它肿瘤患者组及健康对照组(P<0.001);鼻咽癌晚期者血浆及PBMC EBV-DNA拷贝数均明显高于早期组,且以PBMC中EBV-DNA拷贝数增高更为明显。结论荧光定量PCR检测血浆及PB-MC EBV-DNA是一种敏感而可靠的实验方法,对鼻咽癌的诊断具有重要价值。Objective To explore the clinical value of the qualitative determination of EBV-DNA in blood serum and PBMC.Methods Fluorescence quantitative PCR was used to determine the content of EBV-DNA in blood serum and PBMC of 56 cases of nasopharyngeal carcinoma（NPC）,60 cases of xenome and 60 cases of healthy cases and to find the differences in the positive rates detected in EB,the content of copied EBV-DNA and the content of EBV-DNA of patients with NPC at different stage between the three groups.Results The positive rate detected in EBV-DNA in blood serum of patients with NPC was 73.2%,obviously higher than those in the xenome group（23.3%）and healthy group（11.7%）respectively（P〈0.01）.The positive rate detected in EBV-DNA in PBMC of patients with NPC was 89.3%,obviously higher than those in the xenome group（8.3%）and healthy group（5.0%）respectively（P〈0.01）.The content of the copied PBMC EBV-DNA of patients with NPC at the late stage was obviously higher than at the early stage,with the content of the copied PBMC EBV-DNA in PBMC higher.Conclusion Fluorescence qualitative determination of EBV-DNA in blood serum and PBMC is a reliable experiment and has high clinical value for diagnosing NPC.
