VEGF-C mRNA表达与腋淋巴结阴性乳腺癌预后的关系

摘　　要：目的探讨VEGF-C mRNA的表达与腋淋巴结阴性（axillary node negative,ANN）乳腺癌患者预后的关系。方法收集ANN乳腺浸润性导管癌50例,用原位杂交方法检测乳腺癌组织中VEGF-C mRNA表达;用免疫组化EnVision法检测淋巴结中广谱角蛋白（AE1/AE3）表达以判定微转移;同时检测ER和c-erbB-2蛋白在肿瘤组织中的表达情况。所有病例随访60~112个月。结果 11例患者腋窝淋巴结发现微转移灶,微转移率为22%（11/50）,有微转移者其原发灶VEGF-C mRNA阳性表达率（45.45%）高于无微转移者（25.64%）,差别具有统计学意义（P=0.049）;且VEGF-C mRNA表达与ER和c-erbB-2蛋白表达无相关性。在50例ANN乳癌患者中,VEGF-C mRNA表达阳性和阴性患者的5年生存率分别是53.33%和77.14%,差异有统计学意义（P〈0.05）。结论 VEGF-C mRNA的表达与ANN患者淋巴结微转移及术后5年生存率密切相关。检测癌组织中VEGF-CmRNA的表达可能为预测ANN乳腺癌患者的预后具有重要意义。Objective To study the expression of VEGF-C mRNA and its relationship with the prognosis of axillary node negative（ANN） breast cancer.Methods The espression of VEGF-C mRNA was investigated by in situ hybridization in 50 cases of ANN breast cancer.The lymph nodes micrometastasis was examined by monoclonal antibodies against cytokeratin（AE1/AE3）.The expression of ER and c-erbB-2 were immunohistochemically detected by EnVision method.All the caeses were followed up range 60 to 112 months.Results Micrometastasis of lymph nodes were detected in 11 cases,micrometastasis rate was 22%（11/50）.A significant difference was found between lymp node micrometastasis positive group and lymp node micrometastsaes negative group in the expression of VEGF-C mRNA（45.45%and 25.64%,P=0.049）.The expression of VEGF-C mRNA was not correlated with the expression of ER and c-erbB-2.The five years survival rates were showed 53.33% and 77.14% in patients of the VEGF-C mRNA positive and negative（P〈0.05）.Conclusion A close correlation was found between VEGF-C mRNA expression and lymp node micrometastasis and the five years survival rates in ANN breast cancer.It is helpful for predicting the risk of ANN by examing the expression of VEGF-C mRNA.
