肝损伤患者外周血单个核细胞中白介素5 mRNA的表达及其临床意义

摘　　要：目的探讨肝损伤患者外周血单个核细胞(PBMCs)中IL-5 mRNA的表达水平及其意义。方法采用RT-PCR法检测64例肝损伤(36例慢性肝病、17例急性重型肝炎、11例原发性肝癌)患者和18例健康对照者(对照组)PBMCs中IL-5 mRNA的表达水平,并分析IL-5 mRNA表达与血清胆红素、内毒素血症及患者预后的关系。结果慢性肝病、急性重型肝炎、原发性肝癌患者PBMCs中IL-5 mRNA的表达水平〔分别为(0.1611±0.1035)、(0.2838±0.0537)和(0.3313±0.1348)均显著高于对照组(0.0223±0.0047),差异有统计学意义(P<0.01);而急性重型肝炎和原发性肝癌患者IL-5 mRNA表达水平亦显著高于慢性肝病患者,差异有统计学意义(P<0.01)。64例肝损伤患者中,血清高胆红素(≥342μmol/L)水平者PBMCs中IL-5 mRNA表达水平显著高于低胆红素(<342μmol/L)水平者,内毒素(+)者IL-5 mRNA表达水平显著高于内毒素(-)者,死亡者IL-5 mRNA表达水平显著高于存活者,差异均有统计学意义(P<0.01)。结论检测PBMCs中IL-5 mRNA的表达水平,对于了解肝损伤患者的肝损害程度、早期诊断和预后具有重要的意义。Objective To investigate the expression levels of interleukin-5（IL-5）messenger RNA （mRNA） in peripheral blood mononuclear cells （PBMCs） of patients with liver injury.Methods Expression level of IL-5 mRNA in PBMCs was measured by RT-PCR in 64 patients with liver injury 〔36 patients with chronic liver disease （CLD）,17 patients with fulminant hepatitis,11 patients with primary carcinoma of liver〕 and 18 normal controls （control group）.The relationship between the expression of IL-5 mRNA and serum bilirubin,endotoximia and prognosis was analyzed.Results The levels of IL-5 mRNA of CLD （0.1611±0.1035）,fulminant hepatitis（0.2838±0.0537）,primary liver carcinoma patients（0.3313±0.1348）were significantly higher compared with that of NCs（0.0223±0.0047）,（P0.01）;and its expression in fulminant hepatitis,primary liver carcinoma patients were also higher than that of CLD（P0.01）.In 64 patients with liver injury,the expression levels of IL-5 mRNA in PBMCs were correlated significantly with the concentration of serum bilirubin,with its expression in Serum bilirubin （≥342 μmol/L）significantly higher than that of low serum bilirubin.The expression level of IL-5 mRNA in patients with positive endotoximia is significantly higher than the negative ones.The expression level of IL-5 mRNA in the deaths is significantly higher than that in the survivals.All the difference is significant（P0.01）.Conclusion it is significant to measure the level of IL-5 mRNA in PBMCs for the understanding of liver injury degree,early diagnosis and prognosis.
