肾上腺素脑室冲洗对脑室出血时颅内压的影响

摘  要:目的通过运用肾上腺素进行脑室冲洗,以探讨其对脑室出血时颅内压的影响。方法测定74例脑室出血患者运用保守、侧脑室引流、肾上腺素脑室冲洗方法后的血压、心率、脑脊液引流量和颅内压。结果保守治疗组高颅内压引起血压升高、心率下降,侧脑室引流后血压下降、心率升高;侧脑室肾上腺素冲洗后出现脑脊液引流量下降、颅内压无明显变化。结论侧脑室引流可以控制颅内压,逆转其心血管的反应;肾上腺素脑室冲洗可减少出血和脑脊液的分泌,颅内压无明显变化。Objective To investigate the effects of intracranial pressure about irrigation of the ventricle hemorrhage with epinephrine. Methods The blood pressure, heart rate index, cerebrospinal fluid volume and intracranial pressure of 74 patients with intraventricular hemorrhage were determined by conservative lateral drainage, irrigation of intraventricular with adrenaline. Results The increased blood pressure and faster heart rate were caused by continuous ventricular drainage with spontaneous intraventricular hemorrhage. The volumes of cerebrospinal fluid were reduced, but no change of intracranal pressure after irrigation of the ventricle hemorrhage with epinephrine. Conclusion Ventricle drainage can control the intracranial pressure, and reverse its cardiovascular response ; adrenal saline rinse of ventricle can reduce bleeding and cerebrespinal fluid secretion, there is no significant changes in intracranial pressure.
